Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) who's services include providing personalized, affordable, high-quality medical, dental, vision and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their individual needs. Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. POSITION SUMMARY: _*
This position reports to the COO and works alongside the CMO and other leadership to build an infrastructure at East Valley to support population health management. This includes assessing and evolving the care delivery model and technology supports to continue to identify patient needs and drive the right resources and care to the right patients. As a key member of the leadership team, this position is leading EVCHC practice transformation to meet the new Medi-Cal focus on population health management; while financially succeeding within their Alternative Payment Methodology environment. This effort includes leadership in initiatives such as the Kaiser Permanente Population Health Management Initiative (KP PHMI). This position will focus on serving all of EVCHC ‘s patient populations in alignment with our mission. The intent of this position is to drive population health management, quality improvement and address health care disparities and equity within EVCHC, while ensuring a positive financial impact to our operations.
ESSENTIAL POSITION FUNCTIONS

  • Actively participate in Kaiser Permanente Population Health Management Initiative (KP PHMI) governance/oversight, design, and development meetings, and communication of PHM solutions back to EVCHC leadership and staff.
  • Actively participate in at least one “test of change” within the delivery model solutions sets. And then lead in the implementation throughout EVCHC.
  • Participate in the PHM technology vendor proof of concept. And then lead in the implementation throughout EVCHC.
  • Lead in review of and feedback on clinical guidelines to adopt and follow within the PHM throughout EVCHC.
  • Provide baseline data for defined PHMI measurement set and report to EVCHC and KP PHMI.
  • Lead the assessment of EVCHC needs and gaps for PHM technology. Based on assessment, decision made on PHM Platform adoption. And lead in the implementation efforts.
  • Ensure the consistent implementation of the EVCHC’s mission, vision and values, including high quality, patient focused health care.
  • Identify core PHM team from current staff to engage in the design and development work.
  • Lead small tests of change around various components of the solutions sets and provide feedback to the appropriate Design Team or participate in the PHM technology vendor proof of concept and provide feedback
  • Data capture and submission of data reporting template
  • Lead efforts in feedback and outcome sessions of the POC pilots; conduct internal gap, technology capability, and readiness assessments as applicable
  • Working closely with IT throughout the design, testing, and implementation of the POC along with the roll out of the full implementation EVCHC wide.
  • Working closely with Finance to ensure the proper documentation, workflow, process & procedures, and other requirements have been created and followed in order to secure maximum allowable reimbursement.
  • Perform other job-related duties, as may be assigned.

POSITION REQUIREMENTS AND QUALIFICATIONS:

  • Must have excellent interpersonal skills and empathy towards patients, as well as have excellent communication skills, critical thinking skills, the ability to handle stressful situations, the capacity to function independently, have varied clinical experience, and the ability to document meticulously.
  • Knowledge of practice transformation components.
  • Proven ability to effect process change.
  • Knowledge of regulatory compliance i.e., NCQA, HIPAA, FQHC, OSHA, CLIA, etc.
  • Knowledge of managed care and health plan requirements for Medi-Cal, Medicare and Covered California.
  • Ability to manage and supervise various positions and relate well to people from diverse ethnic and cultural backgrounds, as well as have a passion for working with at-risk, culturally diverse populations.
  • Basic understanding of information technology and ability to organize, analyze and synthesize complex data from various sources.
  • Able to adapt process improvement in accordance with organization objectives.
  • Willingness to work flexible hours in order to meet the organization’s needs/demands.
  • Bachelor’s degree in health care related discipline required.
  • Master’s degree in Public Health, or related master’s degree is preferred.
  • A minimum of 5 years of progressively responsible management experience in health care industry, preferably in FQHC setting.
  • Computer literate; proficiency with MS Access, Word, and Excel
