Job Description:

Description: The Director of Community Health is responsible for the direct oversight and the personal and professional development of all the Practice Managers for assigned clinics. This job maintains responsibility for the safe and efficient operation of the assigned clinic(s) (including clinical and non-clinical operations).

Typical Duties:

  • Establishes a solid working relationship with all key leadership including other Directors, Practice Managers, Medical Directors, Department Chairs, Nursing leadership and others, to ensure system wide integration and coordination of the clinical and non-clinical operations of all areas.

  • Develops Practice Managers and CHC staff members by defining roles, responsibilities and expectations, assessing and communicating performance and promoting professional growth and accountability. Assists Practice Managers in communicating and aligning work priorities with the organizational Vision, Mission, Values and Service Standards.

  • Leads growth initiatives with a systematic planning process. Collaborates with the leadership team, working strategically to build strong programs in relation to benchmarks and standards. Leads work force initiatives.

  • Establishes objectives and benchmarks to evaluate operational success for region managed and prepares reports to inform management team of status and progress towards goals.

  • Builds and supports effective relationships with and between all individuals including staff, physicians, the community, patients, families, volunteers, and colleagues to ensure the delivery of safe, quality patient care.

  • Creates and sustains a culture based on trust, mutual respect, and common understanding of needs. Supports and implements process improvement activities that enhance clinical outcomes, improve patient satisfaction and improve operational efficiency to manage costs more effectively.

  • Ensures financial stewardship for assigned region for operational and capital acquisition processes. Maintains an adequate system for internal control over financial interactions.

  • Leads innovation in care delivery model design and in processes that ensure patient safety.

  • Designs and oversees an organizational structure that ensures effective delivery thus enhancing the patient family experience.

  • Performs other job related duties as assigned.


Qualifications:


    Required Education and Experience:
  • Bachelor’s degree in Health Services Administration, Business Administration, Nursing or related field of study from an accredited University.
  • 3 plus years of experience specific to an ambulatory setting.
  • 5 plus years in a healthcare supervisory role.
  • Preferred Education and Experience:
  • Master’s degree in Health Services Administration, Business Administration, Nursing or related field of study from an accredited University.
  • 5 plus years of management experience in a healthcare setting.
  • Required Licensure/Certification/Specialized Training:
  • 3 plus years of experience with physician productivity and professional fee billing.
