FAIRHOPE Hospice & Palliative Care, Home of The Pickering House is seeking a Clinical Director to be responsible for directing and leading the Interdisciplinary team (IDT). Help meet the needs of hospice patients and families in a manner consistent with the guidelines and policies of FAIRHOPE. Serve as the agency resource for patient care and ensures positive working relationships for coordination of care between the family, physician, and others involved in the patient’s care.
We believe in a professional team approach to promote the philosophy of palliative and hospice care concept. This position works in direct collaboration with the President & CEO and the Leadership team.
The qualified candidate must demonstrate strong communication and social interaction skills, as well as, be a compassionate individual to work with other professionals and patients and families who face a vulnerable time in their lives. The ability to manage time efficiently. Candidate must be able to analyze and implement proper action is essential. Extensive computer skills required. Must be knowledgeable and proficient in Microsoft Office.

  • *This position is full-time exempt (salaried). Requires Bachelor’s degree or equivalent in nursing from a program accredited by the National League for Nursing preferred. Management experience in end-of-life care; hospice and palliative care or home health related organization preferred.

What you'll do:
CORE FUNCTIONS
CLINICAL RESPONSIBILITIES

  • Administers, coordinates activities, and directly supervises managers of Clinical teams for Home Care, Inpatient Care, and Social Services departments.
  • Reads, understands, and initiates practices into action to meet the requirements of the Hospice Conditions of Participation for Medicare, applicable accreditation standards, and state or other rules and regulations as applicable to the Hospice program.
  • Ensures hospice services are available 24 hours a day/7 days a week.
  • Supervises patient and family care as specified by the plan of care (POC), assessing appropriateness, continuity, service, and quality of care.
  • Oversees documentation in the patient’s clinical record for accuracy and regulatory requirements.
  • Oversees documentation of the patient-centered plan of care and the implementation of interventions for patient care, including:
  • Changes in the level of care;
  • Medication changes;
  • Changes to visit frequency; or
  • Need for emergency transportation.
  • Participates in care delivery for patients as needed including conducting admissions, re-certification visits, routine visits, and discharge visits. Participates in after-hours shifts as needed including primary and back-up.
  • Oversees the consultation process between the FAIRHOPE Medical Director/Assistant Medical Directors and the members of the Interdisciplinary Team (IDT), assists in requesting appropriate care orders and ensures attendance given to identified patient needs if patient’s attending physician is unavailable.
  • Leads IDT meetings and ensures the following actions are taken for IDT meetings:
  • Patient care needs are identified and discussed with the full complement of the IDT.
  • All required members of the core care team are present and engaged.
  • Required documentation is completed during and at the conclusion to capture the discussion and outcomes of the meeting.
  • Patient re-certifications are discussed and completed in a timely manner.
  • Facilitates and exhibits accurate and timely communication ensuring continuity of clinical workflows between medical records, billing/administration, intake/admissions, clinical staff and leadership team.
  • Actively collaborates with Director of Quality & Regulatory Compliance regarding QAPI activities and provides feedback and input into Performance Improvement projects and initiatives.
  • Participates in Performance Improvement programs.
  • Analyzes and reports on quality metrics including Hospice Item Set (HIS), CAHPS, and symptom management outcomes.
  • Participates in budgeting process and oversight of Clinical department’s financial status.
  • Ensures maximum utilization of resources.
  • Responsible for the management of pharmacy, medical supplies, durable medical equipment, mileage, and labor for department to meet budget expectations.
  • Reviews and approves invoices related to clinical operations.

Management/Leadership

  • Serves as member of the organization’s Leadership team. May be designated the administrator or alternate administrator for the licensure of the program based on state requirements.
  • Is flexible and available to clinical and customer service teams 24/7. Assigns appropriate designee when unavailable.
  • Establishes and maintains effective working relationships, communication, problem resolution and networking with leadership/staff, other agency teams, and suppliers/vendors.
  • Is compliant with internal and external regulatory standards including local and state health departments, Medicare, Medicaid, The Joint Commission, licensure and agency policies and procedures to assure readiness for inspections, both announced and unannounced.
  • Collaborates with Director of Quality & Regulatory Compliance in developing, maintaining, and implementing Clinical and Social Services practices, procedures and standards. Ensures compliance with same.
  • Participates in the ongoing development, recommendations, and activation of agency Emergency Operations Plan as emergencies may arise.
  • Assists in the development of organizational goals and development, recommendation and administration of organization policies and procedures.
  • Participates in the selection, orientation, ongoing staff training, job performance evaluation and discipline of Clinical and Social Services team members. Follows state regulatory requirements for supervision of nursing services.
  • Possesses ability to manage people and exhibits strong conflict resolution skills.
  • Has strong organizational and system skills, as well as ability to manage multiple tasks and prioritize as necessary.
  • Directly responsible for managing time and attendance (scheduling of staff), payroll and tracking benefit time of Clinical and Social Services team members.
  • Completes and administers performance evaluations prior to anniversary date for all directly supervised managers of Clinical teams for Home Care, Inpatient Care, Home Care Aides, and Social Services departments.
