Citizens Care & Rehabilitation Center, a skilled nursing facility in Havre de Grace (Harford County) is in search of a Director of Admissions/Hospital Liaison.

This position is responsible for the organization, direction and supervision of the admissions department. Responsible for census building and marketing initiatives. Coordinates the admissions process with prospects and internally.

Essential Responsibilities/Duties:

· Develops, supports and demonstrates practices consistent with the mission, philosophy and standards of the admissions department and the facility

· Develops, reviews and revises policies and procedures for the admissions department

· Trains, manages and evaluates the performance of department personnel

· Acts as an advocate for the facility by marketing available services via social media and company website

· Fosters relationship building and maintains a good rapport with transferring institutions in order to meet census building initiatives

· Maintains an accurate facility census and knowledge of available beds; ensures a census of 93% or above

· Ensures prompt and professional response to eDischarge; internet based discharge planning solution

· Ensures positive first impression with prospects via telephone and in person contact

· Conducts facility tours

· Responsible for interviewing potential residents, guests and their representatives to collect accurate data on admission inquiries, including screening for proper placement in the facility

· Explains facility philosophy, policies and procedures and admission agreement to potential and new residents, guests and their representatives

· Coordinates all medical requirements for admission of residents and guests

· Verifies resident’s and guest’s medical and financial eligibility prior to admission

· Works cooperatively with Business Office

· Ensures that decisions to decline potential residents and guests are made in cooperation with Administration

· Communicates admission inquiry data verbally and in writing to the nursing department and Administration timely

· Assigns appropriate rooms to residents and guests in consultation with the nursing department and administration

· Notifies appropriate facility departments when a resident or guest is admitted

· Ensures demographic information is entered into PCC prior to admission

· Ensures all administrative paperwork with new resident, guest or their representatives is completed within 72 hours of admission

· Refers admission problems to appropriate individuals

· Communicates status of hospital bed hold and re-admission with Administration

· Regularly reviews status of prospective residents and guests on waiting list

· Accepts requests for resident and guest accommodations in an accurate and organized manner

· Reviews and approves payroll through timekeeping/payroll system

· Attends In-Service training to maintain and improve competencies

· Other duties as assigned

Education/Experience:

Bachelor’s degree in Business Administration, Social Work or related field is preferred. Five to eight years admissions experience in long term care, or equivalent combination of related education and experience.
