This position is full-time, benefits eligible, and located in Urbana, West Liberty, and Bellefontaine, Ohio.

Primary Responsibilities

Individual:

  • Provides individual, group, and family, case management and/or counseling under clinical supervision as needed or requested that meets quality requirements set by the Agency and that is within the limits of services permitted by position and scope of practice.
  • Completes intake/diagnostic assessments on all new clients, under clinical supervision, and assigns each client to a case manager as assigned.
  • Provides court representation, community probation, and conditional release services as assigned.
  • Maintains current knowledge of community resources as evidenced by current and appropriate referrals documented in the client records
  • Provide only those services that have been authorized by Utilization Management and provide services within the authorized frequency and duration of services. Request additional services when initial authorization has been met.
  • Individualized Service Plan and Discharge Criteria with specific goals and objectives that are measurable are developed with the consumer and signed at time of the initiation of services and at time of each update.
  • Assessment, Individualized Service Plan, Progress Notes and other documentation clearly identifies consumer's input in services by having consumer's and parent's/guardian's (if applicable) signatures when clinically appropriate.
  • Complete progress notes at the end of each session and turn in within 24 hours of completion.
  • Notify consumer and referral source (when applicable) prior to completion of the discharge summary and document this notification on discharge within 30 days of last contact.
  • Oversees supervision in MH Residential Homes working closely with the Housing Managers.
  • Coordinate placements of new clients in the home with Housing Managers.
  • Develops and maintains quality, accurate clinical charts for all clients served.
  • Develops and maintains positive collaboration with other social service agencies serving clients. Maintain knowledge and familiarity with referral sources within the community. Provide consultation when needed.

Supervisory

  • Provides administrative and clinical supervision for Community Support Specialists and Community Living Assistants:
  • Coordinates and reviews all referrals and makes dispositions appropriate to CSP services.
  • Reviews charts for 100% compliance with record standards at least once each quarter or more often if needed.

Education, Certifications, and/or Experience

  • Education: Bachelor's Degree in human/social service field
  • Experience: At least 3-5 years in mental health environment working with the SMD population
  • Certification: LISW, PCC, LPCC, preferred. LSW or LPC, Required. First Aid; CPR; de-escalation
  • Other: ODMH licensure and certification requirements; valid Ohio drivers' license; no more than two moving violations within the past year; evidence of liability automobile insurance.
