Crisis Case Coordinator –
Youngstown, OH


Salary $:
$16.000 - $18.00 per hour depending on experience, includes a comprehensive, competitive benefits package. .

Shift: Full-time; 40 hours per week position, M-F, mostly day shift but may require late evenings or weekends based on client needs

Responsibilities:

  • Assesses the after-care and community needs of clients and contributes to the development of ITP
  • Provides individual therapeutic interactions to clients
  • Provides services to clients with the aim of crisis stabilization
  • Provides ADL prompting, coaching, and teaching as necessary
  • Makes appropriate decisions pertaining to clients’ need for additional services, resources or more restrictive settings
  • Provides appropriate treatment interventions, techniques, advocacy activity and frequency of contacts
  • Pursuit of collateral contacts to gather additional psychosocial information when indicated
  • Adherence to agency’s risk assessment/management protocols
  • Completes Intake/needs assessment, progress notes, discharge paperwork, and referral forms appropriately
  • Conforms to Program, OMHAS, CARF and third-party payer standards and obligations
  • Adheres to the Agencies 48-hour documentation policy
  • Documented daily billable services with each client residing at the Crisis Center with an expectation of at least 9 individual interactions daily.
  • Regular interactions with Program Manager and clinical staff to discuss cases and discharge plans
  • Coordinates after-care and ongoing support with other service providers
  • Gathers collateral information and input for discharge planning from family and significant others
  • Maintain ongoing relationships with community resources and other providers for potential referrals
  • Participates actively and productively in agency meetings and supervision sessions
  • Introduces and/or implements new ideas for improving the quality of care
  • Attentive/responsive to feedback from clients, significant others, community partners/members
  • Shares skills, knowledge, expertise with other agency staff (consultations, workshops, team meeting)
  • Advances own level of skill and competency through participation in continuing education opportunities
  • Works to gather and improve knowledge of community resources to benefit clients after-care plans
  • Other duties as assigned by supervisor.


Qualifications:

  • High School Diploma & 3 years’ mental health experience required; bachelor’s degree within related field preferred.
  • 3 years of previous mental health or community support/case management experience required.
  • Experience with severe and persistent mental health diagnoses, substance use disorders and dual diagnosis.
  • Knowledge of community resources is critical.
  • Ability to use a desktop computer, electronic health record for documentation, a landline telephone and access to a cell phone (agency or personal) for accessibility by staff and clients. Experience with telehealth video platforms (Google Meet) and EHR (Credible)

Miscellaneous requirements: Criminal Background Check, CPR/First Aid certified; clean driving record; TB test, able to pass drug screen; completion of agency My Learning Pointe required courses. Enthusiasm, initiative, self-motivated, organizational skills, ease in establishing rapport, ability to effectively communicate with the population, cultural sensitivity, willingness to provide services in the community including client homes.
