Essential Functions: * Serves as the single point of contact for information specific to the court's disposition for eligible members.- Coordinates court ordered evaluations and treatments.
- Communicates court related follow-up/requirements to the interdisciplinary team and collaborates with the interdisciplinary team to achieve optimal resource outcomes.
- Responsible for ensuring required court paperwork is complete and submitted in a timely manner.
- Provides support to care and case managers regarding the coordination of care plans for members by utilizing social service expertise to evaluate the members need for alternative services and third party intervention.
- Assist in coordinating transfers for members under court-ordered treatment to Arizona Long Term Care System (ALTCS) or another Regional Behavioral Health Authority (RBHA).
- Organizes and executes annual medical record review audits for court ordered members – prepares audit tools, conducts record reviews to ensure requirements are met for court ordered monitoring and delivers summary and feedback to provider partners for CQI.
- Represent Care1st at state regulator, workgroups and stakeholder meetings that involve COT
- Develops and maintains relationships with external stakeholders such as judges, court and provider personnel.
- Assists in the development and execution of state contract deliverable : COE/COT Advisory Board
- Prepares and submits a contract required monthly deliverable to the state for members on COT
- Conducts research to clarify fine points of requirements for court ordered member processes in various counties, tribal lands, etc
- Conducts outreach to members for engagement in services; assists members in linking to services
- Performs other duties as assigned.Additional
