Established in 2016 in affiliation with Ruth & Norman Rales Jewish Family Services, JFS at Home was created to meet the growing need for quality care at home for seniors. As a home health agency licensed in Palm Beach County, JFS at Home provides private duty homecare, senior companion services, and geriatric case management, for clients residing primarily in South Palm Beach County. Certified Nursing Assistants and Home Health Aides assist clients with personal and physical care, while senior companions provide socialization and recreation for clients that are more independent. Licensed Clinical Social Workers offer fee for service geriatric case management, coordinating care and helping families manage the complex needs of their aging loved ones. JFS at Home is the only non-profit home health agency in South Palm Beach County and is proud to be a financial supporter of Ruth & Norman Rales Jewish Family Services. Our mission is to promote healthy aging by enabling clients to maintain their quality of life while they continue to live at home, safely and with dignity. We reinvest a portion of our profits to make these services affordable for those in need.

Community Liaison

JFS at Home is seeking a part-time Community Liaison to increase client referrals by acting as the liaison between our agency, our community and referral sources. Community Liaison will market our services and engage in activities aimed at building our client census, by drawing upon their established relationships and cultivating new ones with key members of the senior care community in South Palm Beach County. Must have Book of Business for S. Palm Beach County.

Duties:

• Expand our network of referral sources throughout the Boca Raton, Delray Beach, and Boynton Beach area by engaging in community education, visits to senior living facilities, relationship- building, and cold calling
• Draw upon current contacts and cultivate new relationships with referral sources including executive leadership of senior living communities, rehab facilities, physicians, nurses, senior centers, etc. in order to increase awareness about JFS at Home and drive new client referrals
• Consistently refer clients with the goal of growing client census in Boca Raton, Delray Beach, and Boynton Beach

Requirements and Qualifications:

• Must be well connected in the home health industry with physicians, senior living communities etc.
• Proven track record of consistent referrals to private duty homecare
• Experience providing community education in the geriatric community
• Knowledge of private duty homecare practices

• Ability to pass level 2 background screening and drug screening (cost covered by JFS at Home)
• Must have reliable transportation with current insurance coverage and a valid driver's license
