POSITION SUMMARY: The Community Health Worker Team Lead (CHWTL) provides coaching, and direct support to the CHW team. The CHWTL provides leadership to other CHWs through training, mentoring, and case conferencing. The CHWTL carries a caseload and supports patients and their social and health needs to navigate and access community resources, social services, and medical systems. This role collaborates both internally and externally and will partner cross functionally with team members including medical providers and case managers. The CHWTL demonstrates an advanced level of communication and interdisciplinary collaboration, professionalism, and systems-based thinking with an emphasis on the importance of patient-centered care, education, and organizational involvement to meet the team’s identified goals. The CHWTL will effectively communicate feedback and provide ongoing coaching and mentoring to staff while supporting a learning environment to advance team skills and promote professional development .

Essential Duties and Responsibilities:

  • Assist with training CHWs regarding program enrollment/referral/discharge processes and CHW database systems.
  • Ensure quality of data collected by CHW team, ensure work is completed accurately and in a timely fashion as needed
  • Provide Manager with feedback regarding the Community Health Workers' daily responsibilities and progress
  • Assist with weekly team meetings
  • Operate as a timekeeper for CHW’s in Kronos

*Provides patient care and/ or support activities appropriate to ages served; primarily adults ages 18-65 or geriatric patients ages over 65. May also include care for infant age 0-1 year, child ages 1-12 years or adolescent ages 13-17 years.
