CCS/NW is currently offering a $1000 hiring bonus to fill this F/T, Community Health Worker position, until position is filled.
Hiring Bonus Criteria: $500 on first paycheck and $500 upon successful completion of a 90-day New Hire Introductory Period.

POSITION DESCRIPTION:
The Community Health Worker (CHW) will provide culturally responsive health promotion and care coordination to families and individuals residing in Catholic Community Service housing and connected communities. As a trusted member of the community and/or someone with an unusually close understanding of the community served, the CHW serves as a liaison between health/social services and the community to facilitate access to services and improve the quality and of service delivery. The CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, problem solving , social support and advocacy. The CHW will collaborate with health and social service providers to reduce silos and offer systems navigation. This position is part of a collaboration between CCS and Catholic health-care organizations.

  • Builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, problem solving, social support and advocacy.
  • Assists and advocate for under-served individuals or communities to receive services and resources to address health needs.
  • Collaborates in the review and/or development of culturally and linguistically appropriate health promotion materials.
  • Assists with providing culturally appropriate health self-management education delivered through classes, workshops, outreach events, support groups or one-on-one interactions.
  • Builds relationships with clients by asking appropriate questions in a compassionate manner and uses good listening skills.
  • Maintains and strengthens relationships with key community members and working partners.
  • Documents work thoroughly and accurately in program applicable documentation platform(s).
  • Works collaboratively and appropriately with health-care providers and team members:
    • in a variety of settings (including but not limited to hospitals, outpatient clinics and other medical environments)
Qualifications:
  • 1 year of relevant community experience.
  • Experience working collaboratively within a team environment.
  • Ability to work independently & as a team member.
  • Ability to communicate effectively with a wide range of people from diverse communities.
  • Excellent interpersonal and listening skills.
  • Ability to communicate effectively one-on-one and in groups.
  • Have ability to establish effective working relationships with neighbors, coworkers and staff from other agencies.
  • Ability to use Microsoft office.
  • WA State Driver’s License, reliable transportation & current auto insurance.
  • Applicant must successfully pass required background clearances prior to an offer of employment.
  • Ability to work both independently and as a member of a team.
  • Demonstrate the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.
  • Support and uphold the mission, beliefs and values of Catholic Community Services and Catholic Housing Services.

PREFERRED:
Bi-Language – Spanish highly preferred.
Experience working in medical or social service environment.
