An active Community Health Worker certification is required for this role, in addition to an experience within healthcare, human services, community-based services, and/or hospital discharges. What You Will Do

The Community Health Worker (CHW) is a part of Baltimore Washington Medical Center’s (BWMC) Get Well Program, which partners with adult clients referred after an emergency room visit or inpatient stay at BWMC. The CHW works collaboratively with the client and their care team to identify and address gaps in care and social determinants of health needs while improving access to appropriate community resources. The CHW will be engaging with clients in the home and hospital setting, providing program liaison and in-home health coaching services.

As a Community Health Worker you will:

  • Support shared decision-making in the development and execution of a person-centered care plan while providing timely access to care and connections with community resources.
  • Provide individualized support to clients in the development of health care strategies and identifying community resources to address health disparities and barriers to care.
  • Complete contact requirements, monitor the plan of care, document progress or lack of progress towards meeting goals, and assist with client access to appropriate community resources to meet their health and safety needs.
  • Coordinate across multiple settings including the hospital, medical offices, and outpatient services, guiding clients to play an active and informed role in self-advocacy and care management.
  • Provide liaison activities in the hospital setting, when assigned, to promote client engagement and successful program enrollment post-hospital discharge.
  • Work closely with the RN Clinical Care Coordinator and other team members to provide guidance to the client and/or family/caregiver for effective care transitions, improved self-management skills and enhanced client-practitioner communication.
  • Promote a holistic approach to identifying and meeting clients’ needs using motivational interviewing and trauma informed care techniques when communicating with clients.
  • Work autonomously utilizing the Coleman model as the basis for client/caregiver interactions, educating on care management red flags as appropriate.
