POSITION TITLE: Community Health Worker

PROGRAM NAME: West Salem Clinic, Total Health Community Clinic, Homeless Outreach and Advocacy Project (H.O.A.P.), & Health Shelter Outreach Transitions (H.O.S.T.)

LOCATION: 1233 Edgewater St. NW, 180 Atwater St. N Monmouth, 694 Church St. NE, & 1143 Liberty St. NE

EMPLOYMENT STATUS: Full-Time

SUPERVISOR: Clinical Services Manager & Infection Control Officer, and the HOAP Program Manager or the HOST Program Manager.

FUNCTION:

Community Health Workers (CHW’s) works in partnership with the primary care team and Transitional Programs to assist patients and their families to improve holistic individual and community health. The CHW identifies barriers that prevent full access to health care, seeking solutions and services tailored to the needs of each patient. The CHW will facilitate access to doctors, pharmacies, health services and other social services that will assist people to improve knowledge about their health and understand the key factors of managing the treatment of the disease by themselves. The CHW works directly with clients, families, and health care personnel to support clients with serious illnesses and social determinants of health that impact health outcomes. The CHW will work as a member of a care coordination team and will support clients in implementing their individual care plan when needed. The CHW administers various health screenings and uses brief interventions to achieve better health outcomes for the patient. The CHW will be working with client health information and must be able to maintain strict confidentiality using the HIPAA requirements. This position requires a flexible, self-directed person, who is excellent at handling situations where all the details may not yet be available or determined and can search for the information that is needed. The CHW must be an excellent communicator using motivational interviewing skills when interacting with patients/clients as well as professional health care employees and community partners. They must be able to empower the clients to manage their health.

GENERAL DUTIES:

  • Consistently performs specific assigned daily and general duties; meets deadlines.

  • Meets established attendance criteria and starts work promptly.
  • Knows and consistently implements the organization's mission and all approved policies, protocols, and procedures.
  • Regularly supports compliance and accreditation efforts as assigned including, but not limited to OSHA, HRSA, FTCA, Joint Commission, HIPAA and the CCO’s.
  • Respects and acknowledges the organizations commitment to cultural diversity, which is expressed through behavior, language, and actions.

  • Consistently interacts with clients and staff in a manner that reflects favorably on the organization and promotes teamwork.
  • Keeps all Expirable documents up to date with Human Resources as applicable (i.e. Driver's license, auto insurance, CPR card, Food Handler card, immunizations, personnel update, annual safety quiz, other annual trainings as assigned, and other items as they are identified).
  • Supports efforts to create a health system that recognizes the complex interaction of mind and body through the integration of behavioral health principles into the everyday practices of healthcare delivery and health promotion.
  • Supports efforts to improve the culture of patient/client/employee safety as an essential component of preventing or reducing errors and improving overall health care quality.

SPECIFIC DUTIES:

  • Empowers patients/clients to navigate the healthcare system on their own and become advocates for their own care. Models behaviors for patients such as checking on appointments or arranging assistance including transportation.
  • Uses motivational interviewing and brief interventions when assisting patients in understanding their diagnosis, treatment options, and resources available. Offer support during medical appointments or addictions recovery.
  • Actively builds awareness of CHW services among the health care team by coordinating and monitoring patient/client care through the Care Continuum. Identifies barriers, obstacles, and social determinants of health for improving patient health by providing information and resources for specific needs. Serves as a single point of contact for primary care physicians, patients, and caregivers to ensure access to clinical and support care services. Connects participants with social services such as housing, energy, food, clothing, showers, and laundry services.
  • Maintains communication to identify patients/clients who are “at risk” for barriers to treatment and in need of patient navigation services. Collects data and tracks interventions and outcomes to support strategic planning processes.
  • Work closely with Transitional Program Coordinators to help support the objectives of the Transitional Program Day Centers and the needs of any Peer Associate or consumer volunteers.
  • Assist individuals with establishing a support network and addressing physical, social, and emotional needs.
  • Participate, as needed, in NWHS medical and street-based outreach initiatives working directly with patients/clients in the field to identify at-risk and vulnerable individuals who are disconnected from medical and/or social services.
  • Provides information on various patient education programs and tools within the community. Provides appropriate teaching, outreach, and education to patients and families.
  • Recognizes and reports signs and symptoms of abuse and neglect to physician(s), and the appropriate Director(s) or Program Manager(s). Completes all mandatory reporting requirements, as needed.
  • Stay informed of changes in the healthcare organization, seek current information about available patient/client resources, and maintain current disease information for patients.
  • Maintain confidentiality; information obtained must be kept confidential and secure. It is important that CHW stay organized and use a systematic approach to information management.
  • Assist with meeting the Clinical Quality Measures by understanding benchmarks and extract data from the EMR to meet and support compliance requirements. Accurately documents the administration of care in the patient recorded in a timely manner.
  • Completes other patientcare or program objectives as requested by the Clinical Services Manager & Infection Control Officer, and the HOAP or HOST Program Managers.

QUALIFICATIONS:

  • Bachelor's Degree in related health care field or experience preferred.
  • Minimum 1-year clinical specialty area experience preferred
  • Must be able to build effective working relationships, solve problems, direct patients to resources and manage information. Understand your role’s boundaries and be able to clarify them to your patients so you can maintain a professional relationship.
  • Excellent phone and computer skills required.
  • Must be able to find creative solutions to patient barriers that require initiative and creative thinking to locate the appropriate resource.
  • Demonstrated knowledge or experience working with low-income and/or vulnerable populations that have acute/chronic health concerns, mental health conditions, or substance use preferred.
  • Completion of a Health Navigator and Community Health Worker Training course or certification course preferred.

PHYSICAL ACTIVITIES AND REQUIREMENTS OF POSITION:

  • The physical activity of this position may include sitting, stooping, kneeling, crouching, reaching, standing, walking, picking, pinching, typing, and lifting.
  • The worker is subject to inside and outside environmental conditions.
  • Medium work, exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently and/or up to 10 pounds of force constantly to move objects.
  • May involve assisting clients who have ambulatory problems.
  • Must wear scrubs or business casual attire.

Job Type: Full-time

Pay: $18.65 - $27.95 per hour
