This individual will use their expertise and knowledge of community-based approaches to assure patients can access care across the healthcare system and assist patients to overcome barriers to care by assisting with peer education, support, advocacy, problem solving and resource connections. They will work with community-based organizations and health care providers in Missouri to implement shared solutions to improve health outcomes in the communities they serve. Health Quality Innovators (HQI) is seeking an associate consultant/community health worker with strong grassroots connections to support community-based initiatives that improve chronic disease management, increase adult immunizations, and reduce health disparities for people with Medicare.

HQI community health consultants review claims and public health data to identify locations where there is opportunity to improve targeted health outcomes. They build relationships with organizations in these locations, such as Area Agencies on Aging, faith-based ministry networks and multi-agency public health coalitions that conduct health improvement activities, such as vaccination outreach campaigns, hypertension screening events and chronic disease management programs. Consultants seek gaps that HQI can fill, such as by providing supplemental data, and identifying strategies for amplifying impact, such as by facilitating connections to additional partners and resources. Where local initiatives are absent, consultants bring stakeholder organizations (e.g., health department, family medicine association, local government, regional YMCAs, etc.) together in advisory committees to build community momentum for activities that members push out to their grassroots constituents (e.g., local health districts, family practitioners, YMCA branches) for implementation.

As a HQI consultant, you will be part of a team of experienced colleagues who share clinical knowledge, quality improvement expertise and best practices for supporting vibrant community health partnerships. You will be able to strengthen or acquire valuable quality improvement skills that position you for advancement. In addition, because this is a remote position, you will have the flexibility that comes with working from home.

Requirements:


Successful consultants are resourceful, flexible and internally motivated. They manage multiple priorities, use good judgment to keep work moving, and consistently meet deadlines. Other requirements include:

  • Project management skills, including three to five years’ experience coordinating initiatives such as chronic disease management programs or adult immunization campaigns
  • Experience in community work, education, health care, or related field required. Completion of a community health worker certification course, blood-pressure self-monitoring program training, or diabetes prevention program lifestyle coach training also helpful.
  • Experience convening and managing stakeholders, including safety net providers and grassroots groups that serve disadvantaged populations. Familiarity with health equity principles and practices.
  • Ability to interpret data, such as performance on quality improvement measures helpful.
  • Ability to use Microsoft Office tools.
  • Excellent interpersonal communication skills, including cultural sensitivity.

  • Ability to facilitate meetings and make presentations

  • Although this is a remote position, in-state travel will be required for some community outreach activities and occasional meetings.

  • Because we are a federal government contractor, you must be able to document full immunization against COVID-19 or present a valid exemption before you can be eligible for an offer of employment.
