Aging & Disability Services at Catholic Charities offer Information & Assistance, Care Coordination, Crisis Intervention, Options Counseling, and volunteer opportunities for older adults and persons with disabilities.

GENERAL RESPONSIBILITIES:*

  • Health Worker facilitates health promotion workshops with community members to build knowledge and skills related to fall prevention and health self-management. The health promotion workshops are ACL approved evidence-based hprograms, which may include but not limited to: A Matter of Balance (MOB), Bingocize, Chronic Disease Self-Management (CDSMP), Diabetes Self-Management (DSMP), Tomando Control de Su Salud, Fit and Strong, Healthy Steps in Motion, Stress Busting Program for Family Caregivers of Persons with Dementia, Stress Busting Program for Family Caregivers of Persons with Chronic Conditions, and Tai Chi for Arthritis.
  • Organize and facilitate educational activities to provide community members with opportunities to acquire knowledge and skills related to interests and capabilities. Subject areas may include nutrition, health, mental health, personal care, entitlements and benefits, and similar topic areas.
  • Organize and facilitate recreation activities which foster the health and social well-being of community members through social interaction and constructive use of time.
  • Complete health screening and evaluation services that assist individuals in maintaining health through identification of health needs and linkage to health care system. Activities may include organization and facilitation of blood pressure, vision, hearing, podiatry, vaccination, and other health care activity events. This role assists with the coordination of flu shot administration.

ESSENTIAL FUNCTIONS:

  • Identify and conduct outreach to high-risk or vulnerable populations, including members of minority populations, low-income populations, adults living with disabilities, and older adults.
  • Health Worker participates in community outreach and participant recruitment activities, including completing telephone calls, staffing tables at community events and fairs, and delivery of flyers to community partners.
  • Assist with the maintenance of a distribution list for community sites to promote programming.
  • Ensure the timely and accurate completion of all required documentation, including event sign-in sheets, participant forms, and evaluation materials.
  • Maintain a cooperative working relationship with service providers and community organizations to meet client needs.
  • Ensure the privacy and confidentiality of participant level data.
  • Coordinate work schedule for adequate program coverage and utilize agency time and resources effectively.
  • Ability to work independently and as part of a team, bringing concerns or challenges regarding complex issues to the attention of a direct supervisor.
  • Act as an agency ambassador in communities served and attend community groups.
  • Maintain required certifications, including the completions of any recertification or annual training requirements.
  • Participate in agency, funder, and community meetings and trainings as scheduled and required.

Requirements: QUALIFICATIONS:
1. High school Diploma or equivalent required. An Associate’s Degree in a human service related field is preferred. All advanced degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
2. Minimum of one (1) year of experience working with older adults and adults with disabilities is highly preferred.
3. Certification as a lay leader or master trainer in Administration for Community Living (ACL) approved evidence-based programming preferred at time of hire. Certification as a lay leader must be completed within six (6) months of employment (or as offered by funding sources).
4. Strong aptitude for public speaking, including to large groups of people.
5. Strong aptitude for facilitating small group events and keeping participants engaged in the session. Experience facilitating group presentations and groups is highly preferred.
6. Ability to work independently and manage multiple tasks and interruptions.
7. Excellent organizational skills, ability to multi-task, and complete work self-directed.
8. Excellent verbal and written communication skills.
9. Strong interpersonal and rapport development skills with older adults, adults living with disabilities, and community members.
10. Ability to interpret available data and complete reporting for funding sources.
11. Knowledge of local services and supports, agency services and supports, and referral procedures.
12. Work effectively and collaboratively with agency staff, community partners, and other program stakeholders.
13. Ability to work during weekday evening hours and occasional weekends in order to offer programs and services that meet the scheduling needs of the client base.
14. This position requires significant travel to the various Catholic Charities and community host sites based on program and service needs.
15. Strong problem-solving skills with a demonstrated ability to respond quickly and appropriately in times of crisis, and to communicate effectively with clients in difficult and stressful situations.
16. Respect participants’ privacy, dignity, and right to self-determination through empathetic and professional communication and work practices.
17. Demonstrated competency to effectively and constructively engage and provide programming for diverse and vulnerable communities, including clients from a variety of religious, racial, ethnic, and socioeconomic backgrounds.
18. Adheres to a “no wrong door” policy when clients approach the agency for assistance.
19. Proficiency in computer and internet-based programs, including Microsoft Office applications and activity reporting systems.
20. Passage of complete finger-print based background clearance.
21. Catholic Charities, Diocese of Joliet, has adopted and follows the Codes of Ethics established by Catholic Charities USA, the National Association of Social Workers and the American Psychological Association. As an employee of the Agency you are expected to support these codes to the extent doing so is consistent with and allowed by applicable law.
22. A valid driver’s license, reliable means of transportation, and proof of liability insurance is required.
23. Bilingual Spanish language competency highly preferred
