PART-TIME COMMUNITY BASED CASE MANAGER - NORTHUMBERLAND COUNTY
Family Resource Specialist
$500 STAY BONUS

Duties & Responsibilities:
  • Assess and engage families in service and treatment planning based on demonstrated needs.
  • Plan and implement appropriate interventions using evidence based and best practice approaches.
  • Assist families in accessing and navigating community resources.
  • Meet with designated clients for the prescribed number of hours/ days/contacts per week.
  • Maintain open communication with referring authorities.
  • Attend court hearings and provide testimony as required.
  • Submit accurate, required notes within 24 hours of service and complete reports within assigned timeframes.
  • Transport clients utilizing personal vehicle, as necessary.
Requirements:
  • Bachelor's degree or High School diploma and experience working with children, youth, and/or families in a variety of settings or environments.
  • Valid driver’s license, auto insurance and access to a vehicle.
  • Pass current, applicable clearances.
  • Pass pre-employment, post-offer drug screening.
  • Ability to work non-traditional hours including evenings and weekends.
