Overview
Pathways Healthcare is seeking a compassionate, forward-thinking Clinical Supervisor (RN) for our Cranston, RI Center. This Clinical Supervisor will supervise, mentor and coach an inter-disciplinary, home health care team that is treating patients in the Cranston territory. The ideal candidate must be able to motivate a team through positive reinforcement and competence in their role because they will be inheriting a strong, purposeful culture and they are expected to continue to build upon it. This leader must also learn and aspire to be a super-user of all systems (current and future); specifically our electronic medical record (EMR) HomeCare HomeBase.

Office Location: Cranston, RI
Job type: Full-time, Monday-Friday, 8:30-5:00
  • This is a hybrid position with both direct patient care and administrative work.

Responsibilities:
  • Engage, inspire and prepare your team at our daily clinical meetings - three onsite per week and two remote.
  • Review patient referrals to determine home care needs and coordinate appropriate services.
  • Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing plan of care.
  • Review and approve revisions to patients' plan of care via EMR workflows in a timely and efficient manner.
  • Utilize EMR, Email, Skype and other systems to their maximum effectiveness to enhance communication with your clinical staff.
  • Lead and participate in the interviewing, on-boarding, and training of all new hires at your center.
  • Facilitate and perform the planning, implementation and evaluation of in-services and continuing education
  • Assist in the formulation, revision, implementation, and evaluation of company goals and objectives; achieve ultimate buy-in from your clinical team.
  • Create and staff our 24/7/365 on-call schedule and our ambitious weekend operations.
Qualifications & Skills:
  • Must have an active RN license in good standing.
  • BSN degree preferred.
  • Maintains current CPR certification.
  • Minimum of two (2) years of direct nursing experience required
  • Prior supervisory experience preferred.
  • Excellent written and verbal communication.
  • Excels with computers, tablets, smartphones, EMR's, Email and other electronic systems.
  • Ability to build upon a positive, purposeful culture.
