The role you’ll contribute:

Independently conducts in-person and telehealth patient visits in the pharmacist-led, referral based, Medication Therapy Management Clinic. Assures the appropriate selection, dosing, administering, and dispensing of medication orders. Works under a collaborative practice agreement to autonomously manage referred patients. Serves as a subject matter expert in pharmacy discipline and provides guidance across pharmacies and to clinical leadership.

The value you’ll bring to the team:

• Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.
• Recommends formulary alternatives when appropriate.
• Provides drug information and education to medical staff and patients.
• Communicates with physicians and other clinicians as necessary to facilitate care.
• Oversees technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
• Assists with academic functions such as serving as a preceptor for pharmacy students. May float to provide coverage of pharmacy needs. Provides patient counseling when required.

The expertise and experiences you’ll need to succeed:

• Active licensure as a practicing pharmacist in Illinois
• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree
• Completion of PGY-1 Residency or equivalent experience and/or board certification

Preferred Qualifications:

• Completion of a PGY-2 Pharmacy Residency Program with focus in ambulatory care
• Specialty pharmacy, retail pharmacy, or PBM experience of 1+ years
• Training and knowledge of specialty areas (oncology, hepatology/gastroenterology, and asthma/allergy)
• Ability to work effectively with minimal direction and as part of interdisciplinary team
• Previous work in hospital pharmacy setting
