ChristianaCare is currently seeking a Clinical Pharmacist to work a Day/Midnight Shift rotation schedule at the Newark campus.

Rotation includes:

• 6 weeks of 7 on/7 off midnight shifts, followed by 6 weeks of day shift (primarily clinical) with weekend rotation group assignment, repeat rotation. Occasional cross coverage of midnight PTO is possible

• Holiday assignments
  • Holidays that fall during the week of midnights will be assigned holiday(s)

• Weekend rotation group assignment when working 6 weeks of day shift
  • Employee will have an assigned weekend rotation, but will only be scheduled to work based on departmental need

Responsibilities:

• Review and interpret provider's medication orders, evaluate drug efficacy, therapeutic goals and indications, review for contraindications/precautions, the patient's current status of health, and review drug profiles and patient's medical record for drug-drug, drug-food, drug-laboratory and drug-disease state(s) potentials.
• Resolve such problems in consultation with the prescriber and nurse, and clarify questioned orders prior to dispensing the first dose.
• Participate in interdisciplinary rounds for assigned patient care unit in clinical shifts. Provide verbal and written consultations concerning drug therapy evaluations as requested.
• Evaluate specific medication therapies consistent with departmental policy; anticoagulation, aminoglycosides and vancomycin
• Appropriately convert/manage medications per P&T approved pharmacist policy; IV to PO and renal dose adjustment
• Report, record, assess, and evaluate adverse drug reactions.
• Provide accurate, timely, and cost-effective drug information to medical, pharmacy, nursing, and other healthcare professionals.
• Serve as a resource and mentor for the staff pharmacist.
• Support and assist interdepartmental projects such as MUEs, P&T drug monographs, newsletter articles and performance improvement activities.
• Provide educational sessions or materials to patient groups or healthcare professionals as needed.
• Coordinates and supervises work-place activities and resources; promotes professional competence; assures that services rendered and work performed are in accordance with specific standards; advises, assists, and supervises work of pharmacy technicians; assumes final responsibility for accuracy of their work; to include satellite pharmacies as well as sterile product processing in the IV Admixture Service lab, OR pharmacy and ambulatory chemotherapy areas, where the pharmacist is responsible for assuring USP 797 standards are consistently in compliance.

Education and Experience Requirements:

• BS degree in Pharmacy required
• PharmD degree preferred
• ASHP-accredited PGY-1 Pharmacy Practice Residency required; equivalent experience will be considered (3 years of acute care hospital experience)
• Current/eligible for licensure in the state of Delaware
• Board certification preferred
