Job Responsibilities

Case Management/Field:

  • Owns the plan of care for all home care and skilled care clients
    • Makes the initial evaluation visit and regularly reevaluates clients' nursing needs
    • Prepares clinical and progress notes and summaries of care
    • Coordinates and guides care for clients as needed across multiple healthcare providers
  • Owns skilled nursing referral intake and follows up with referral sources, clients and families, other care providers, etc
  • Provides patient care as defined within the scope of the Nurse Practice Act - Ohio Revised Code (ORC)

Administrative/Leadership:

  • Manages all quality improvement initiatives and specialty care programs (e.g., Infusion Services, Dementia Care, etc.)
  • Assists in the development and execution of BrightStar Care’s organizational and clinical goals
  • Maintains Joint Commission accreditation and ensure our agency continues to pass survey
  • Develops, recommends, and administers agency policies and procedures
  • Supervises, educates, and coaches nursing personnel and other field staff
  • Serves as first point of resolution for client complaints or issues with care
  • Plans and implements in-service and training programs
  • Oversees the maintenance of client clinical records, statistics, reports, and records for purposes of evaluation and reporting of organization activities
  • Assures proper maintenance of clinical records in compliance with local, state, and federal laws
  • Stays informed about changes in the field of nursing and home health care
  • Shares new information with appropriate organization personnel
  • Promotes home health referrals in the health care community

Job Requirements

  • Unrestricted and unencumbered license as a Registered Nurse in the State of Ohio
  • Bachelor’s degree in Nursing (must have graduated from a National League of Nursing accredited program)
  • Minimum three (3) years of documented clinical experience with at least one (1) year of case management experience
  • Preferred: At least one (1) year experience in a home health agency
  • Preferred: Policy development experience
  • Strong clinical skills. Self-directing with the ability to work with little direct supervision
  • Impeccable communication and professional skills - ability to read, write, speak and understand English fluently
  • Understand regulatory compliance with state and federal regulations
  • Adhere to HIPAA and maintain client confidentiality
  • Demonstrated ability to supervise and direct staff
  • Possess exceptional nursing assessment skills
  • Proven ability to build quality and implement improvements within the operation
  • Comfortable with technology
  • Excellent customer service skills and ability to deal tactfully with clients, staff, and community
  • Ability to travel within the assigned territory daily including weekends, nights, and holidays
  • Ability to pass drug, background, and criminal record checks
  • Certified CPR and negative TB skin test or chest x-ray
  • Licensed driver with reliable and insured automobile (proof of insurance required)
  • Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
