AGENCY SUMMARY

West Michigan Partnership for Children (WMPC) administers foster care services to children and families in Kent County, MI in partnership with Bethany Christian Services, Catholic Charities West Michigan, D.A. Blodgett—St. Johns, Samaritas, and Wellspring Lutheran Services. WMPC is executing an innovative performance-based funding model through a contract with the Michigan Department of Health and Human Services (MDHHS), utilizing a capitated allocation payment mechanism that allows for savings generated from practice and program improvements to be reinvested into the foster care system. WMPC utilizes software that allows for predictive analytics to inform strategy and practice, and to flag cases that are at risk for further review. WMPC’s mission is to empower communities to create better futures for children and families through innovation and collaboration and it does this through its core values of accountable, data-driven, innovative, trauma-informed, tireless collaboration, and social justice.

POSITION SUMMARY

Provides and facilitates mobile access to services provided by Network180, the community mental health agency, for children and families involved in the child welfare system (either through the Michigan Department of Health and Human Services (MDHHS) or placed in foster care through WMPC). Participates in the planning and implementation of system transformation efforts to increase access of children, youth, and families to clinical, mental health services and supports. Contributes to project leadership pertaining to the Sanctuary Model implementation. The Clinical Liaison has a key role in developing and implementing WMPC’s mission, vision, values, and strategic plan, and models shared qualities of effectiveness. Must live in West Michigan. Position is eligible for a $1000 sign on bonus.

JOB DUTIES

Clinical Services (60%):

• Liaises between the Child Welfare System and Network180 to improve access to mental health services.
• Screens and completes clinical notes for children and families involved in the Child Welfare System and facilitates access to necessary Network180 services.
• Increases knowledge of foster care workforce on trauma, and how trauma impacts youth behavior and mental health. Educates workforce on available tools and services and how to access them (i.e., trauma screening, SED/SUD/DD services, and DHHS wraparound services).
• Coordinates with Network180 to screen and authorize SED waiver services.
• Coordinates with the clinical leadership and Care Coordination Manager to address complex client needs.
• Works with Network180 to identify gaps in the service array and barriers to accessing services.

• Assists and/or provides support to plan and manage service continuum to meet diverse consumer needs.
• Meets independently with providers and facilitates group meetings to manage system services and processes.
• Assists and/or provides support with quality performance audits, case reviews, and system improvement projects.
• Coordinates and provides CAFAS/PECFAS training for WMPC network.
• Researches and assists in implementation of best practices as directed.
• Provides licensure supervisor to limited-licensed MSW staff.

Project Implementation (40%):

• Serves as project liaison for implementation of programs.
• Provides consultation and support to partner agencies.
• In coordination with the Director of Care Coordination and Innovation, creates and refines Sanctuary implementation plan.
• Provides Sanctuary training to WMPC staff and network.
• Works on special projects or committees as assigned.
• Other duties as assigned.

QUALIFICATIONS

Education Level

Master’s degree and three years of progressively responsible experience required; clinical license required. Experience working for a child-placing agency preferred.

Field of Study/Specialty

Social Work, Mental Health, Counseling, or related field.

Knowledge, Skills, and Abilities

• Licensed as a master’s social worker, professional counselor, marriage and family therapist, psychologist, or registered nurse.
• Trained or able to be trained to administer the CAFAS/PECFAS/LOCUS/DECA as a reliable rater.
• Lived experiences with mental illness/developmental disabilities/substance use disorders valued.
• Demonstrated experience working in a multicultural environment
• Knowledge and experience in trauma-informed practices
• Project Management skills
• Data analysis skills
• Technical writing expertise
• Interpersonal skills necessary to lead and interact with a diverse group of individuals and stakeholders.
• Proficient with Microsoft Office products and web-based applications
• Ability to read, write, and communicate effectively in English.

Preferred Qualifications

• Knowledge of Community Mental Health and System of Care for Children with Severe Emotional Disturbances principles/standards, practices, systems (Medicaid, medical necessity, provider network)
• Knowledge of mental health service options within and outside the Kent County region and Michigan Medicaid.
• Bilingual in English/Spanish
