Position: Clinical Leader

Department: Clinical Operations

Reports to: Operations Leadership

Pay Status: Full Time

Work Schedule: Monday-Friday, including call rotation

Job Summary:

The Company is looking for a highly effective and experienced Clinical Leader to join their team in New Hampshire. The Company seeks a Clinical Leader to function under the Agency within the confines of the State Nurse Practice Act and the professional standards as described by the Community. The Clinical Leader assumes the responsibility of health care services provided by all Nurses and healthcare related employees. This includes supervision of off- site and office-based staff, recruiting and retention, development, and implementation of program policies and procedures.

Due to the number of facilities we visit, the COVID-19 vaccine is required for this role as we work with a vulnerable population.

Requirements:

  • Current CPR/First Aid certification
  • Degree in Nursing from an approved school of nursing as determined by the Board of Nursing, required
  • Experience in supervising and developing staff
  • Graduate of an accredited school of nursing, BSN preferred
  • Current License as a Registered Nurse from New Hampshire State Board of Nursing
  • Able to problem solve and work effectively with others
  • Current drivers license and proof of automobile insurance
  • Must be able to occasionally exert 50 to 100 lbs. of force
  • Must be able to work a variable and flexible schedule
  • Communication and interpersonal skills to develop relationships with clients, staff, and providers
  • Experience: 5 years minimum as a registered nurse in an appropriate clinical care setting
  • Nursing skills are defined by accepted nursing standards, preferred. Requires interpersonal skills and ability to manage patient care episodes.

Knowledge, Skills and Abilities:

  • Knowledge regarding home health care and /or public health
  • Excellent administrative, organization and leadership skills
  • Good oral, writing and reading communications skills
  • Be aware of and adhere to all HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this Company.
  • Be respectful of people and their cultural, religious and ethnic differences.

Responsibilities:

  • Participates in on call rotation and in charge of creating the on-call schedule for weekdays/weekends and holidays
  • Completes new employee orientation for clinical staff
  • Determines scope and frequency of services needed based on acuity and patient/caregiver/family needs
  • Provides direct patient care as staffing requires
  • Ensures the consistent availability of all care/services offered by the agency
  • Assesses the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care
  • Completes monthly chart audits of 10% of patient census
  • Utilizes case management approach and referring to other services as needed externally as well as internal company referrals
  • Coordinates regular in-service training for clinical staff
  • Maintains proficiency with the agency’s electronic medical record
  • Recommends and arranges for resources needed to provide care/services
  • Responsible for all QAPI and Quality Improvement methods and consistent preparation for state survey
  • Initiates, develops a plan of care for and supervises each client, coordinating patient care between referral source, physician, family and members of the healthcare team.
  • Initiates, reviews and files all client/employee documentation as confidential documents
  • Routinely supervises, evaluates and confers with field employees, monitoring work performance and providing on-the-job training when appropriate
  • Promotes and/or initiates and directs health education programs, regularly scheduled in service/continuing education programs and state mandated
  • Communicates with the Operations Leader concerning branch compliance and functioning
  • Assumes other responsibilities as assigned by Operations Leadership
  • In conjunction with other members of the branch healthcare team, acts to ensure compliance with all corporate, federal, state and local healthcare policies to ensure quality, efficiency, safety and coordination of all provide services.
  • Provides coding and OASIS support to admitting clinicians
  • Applies specific criteria for admission and re certification to establish appropriate levels of care and the patient’s eligibility
  • Manages and coordinates the provision of quality services to patients from admission to discharge

  • Registered Nurse licensed in the State of New Hampshire
