Description: The Burke Center/ Transitions

  • Residential treatment program is 24 hours a day, 7 days a week facility for persons with behavioral health or co-occurring needs, including intellectual or developmental disabilities. Residential treatment programs provide environments in which the persons served reside and receive services from personnel who are trained in the delivery of services for persons with behavioral health disorders or related problems.
  • · Assist with assuring all new admissions receive an orientation to the residential site includes an overview of the rules and procedures as evidenced by the individual signature that verifies that this information has been reviewed with the individual and that they are in agreement.
  • · Assist with coordinate’s transportation for residents for all related treatment services, medical appointment, long-term treatment facilities and other program related activities.
  • · Assist with all new admissions make application for food assistance within the first week of admission or arrange for individual to receive a food voucher.
  • · Assist with monitoring the computer room, inventory/supply order list.
  • · Assist with assuring that residences are kept clean and are well maintained.
  • · Assist with other duties as needed:
  • · Document the Medication Administration Record (M.A.R.) log is initialed and properly completed.

NOTE: McIntosh Trail Community Service Board conducts background checks on all final candidates. Hiring of applicants is contingent upon satisfactory results of:

Fingerprint Based Background Check of the Georgia and Federal Criminal History Record
Database
Drug Screen
Motor Vehicle Report (MVR) for seven years

TB testing will be required upon appointment. If you have previously tested positive for TB, you will be required to bring a copy of your chest x-ray on your first day of orientation.
