Home Helpers Home Care of Leesburg is currently seeking a passionate person for our Client Care Coordinator position.

As a Client Care Coordinator, you will assist the administration staff with the supervision of our Home Health Services to a variety of clients.

Schedule: 9am to 5pm

Location: You will be splitting your time between working from the office, and site visits with clients.

Responsibilities:

• A Care Coordinator, or Patient Care Coordinator, is responsible for scheduling, organizing and managing all aspects of a patient’s healthcare maintenance and treatment.
• Following up with clients and maintaining a relationship with them.
• Maintain updated Client and Caregiver training information with Administrative Staff
• Attend staff meetings and training sessions as requested
• All other duties as assigned by Administrator and the job description

Qualifications:

• Graduate of an approved school of professional Nursing and currently licensed within the state of VA
• 2+ years of home health care is preferred. Limited experience welcomed!
• Good communication, computer, organizational, and time management skills are essential
• Must have prior client care management experience
• Ability to travel between office location, and client location.
• Maintain a professional appearance and demeanor
• Strong work ethic and a person who truly enjoys meeting new people and understanding their needs.
