Position Profile:

This position involves providing casework services to children placed in the TLP/ILO support systems. This also includes coordinating services and communicating effectively with other treatment team members within Children’s Home, and external individuals and agencies. This position requires the flexibility to address issues as they arise and assist in emergencies or staff shortages, as well as the ability to complete the regular requirements and schedule.


JOB FUNCTION:

Provides direct case management services to assigned clients and families including assessment, service plan development and implementation, court representation, and documentation of services; Provides family engagement and permanency planning to assigned clients; Provides counseling, mentoring, and life skills training; Monitors the safety and wellbeing of assigned clients in TLP/ILO; Assures all medical, educational, and clinical needs are met for all assigned clients; Provides crisis management services in order to increase stability and personal functioning; Consults with TLP/ILO staff, as well as other involved parties in order to plan, provide and monitor effective treatment.


QUALIFICATIONS:

  • Education and Experience: Bachelor’s Degree in Psychology, Social Work or Related Field
  • Must have required DCFS licensure or be able to obtain certification upon hire
  • Two (2) or more years experience in child welfare, preferably experience with emotionally and/or behaviorally disturbed youth and their families.
  • Strong interpersonal skills
  • Maturity and sensitivity to cultural and individual differences in clients, staff and families served
  • Effective oral and written communication skills
  • Ability to work within a treatment team
  • Ability to handle job related manners in a professional, diplomatic and confidential manner.
  • Hearing and speaking ability which allows for effective oral communication of information
  • Ability to physically transport self and clients to various locations
  • Physical and emotional stamina to effectively handle job related issues and stress
  • Ability to physically move kids’ items, including some heavy items to apartments, i.e. furniture.
