Overview
Job Summary:
Provides medical nutrition therapy (nutrition assessment, counseling, education, modified diet formulation, nutrition intervention) for inpatients and outpatients (clinic setting). Maintains a high level of patient of involvement and visibility in patient care areas.
Job Responsibilities:
- Assesses, makes recommendations and plans for patient’s medical nutrition therapy.
- Collaborate with health care providers in a team approach to care for individuals with diabetes and other medical issues.
- Completes accurate documentation, as required.
- Develops and implements nutrition care plan in accordance with standards of care and practice guidelines.
- Provides medical nutrition therapy appropriate to age-related needs and diagnosis/disease.
- Monitors and evaluates patient responses to therapeutic interventions and collaborates with interdisciplinary health care team; provides ongoing assessment and recommendations to the patient and family/caregiver and to the healthcare providers.
- Provide diabetes and self-management education to patients and their families/caregivers following standards of care, per the American Diabetes Association.
- Participates in continuous improvement by monitoring trends in hypoglycemia/hyperglycemia and provide education about diabetes and diabetes management to members of the health care team to improve diabetes care.
- Boone Health has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.

Minimum Qualifications:

Degree: Bachelor’s Degree/Nutrition

Experience: 2-5 years

Supervisor Experience: None required

Licenses & Certification: Certified Diabetes Care and Education Specialists (CDCES) Certification, Licensed Dietitian in Missouri, Registered Dietitian
