Our Lewiston office has a full-time position available for an LPN, LSW or MHRT-C to join our Community Care Team/HOME (Housing Outreach Member Engagement) Team.

PURPOSE: To provide care management service to Community Care Team (CCT) and Housing Outreach Member Engagement (HOME) patients. HOME is a specialized CCT that provides housing outreach for eligible members with long-term homelessness.

ESSENTIAL FUNCTIONS: To collaborate with and coordinate community networks/teams and community based supports and service providers who share resources to ensure patients can access all the available supports the community has to offer, and to reduce unnecessary re-hospitalization and impact on health care services. Services and supports may include, but not be limited to, prevention, wellness, specialty care, behavioral health, transitional care and coordination, and social and community services.

The ultimate objective is to establish a person-centered plan of care that includes member health care data, member health goals, and supports necessary to achieve those goals.

EDUCATION: ActiveLPN or LSW license on the State of Maine Registry. Will also consider MHRT-C.

EXPERIENCE: Home Care experience preferred. Patient care, and Case/Care Management experience also preferred. Knowledge of community & housing resources in Androscoggin, Cumberland, Franklin, Oxford Counties & Mid Coast Maine. Clinical Skills related to Infection Control & Prevention. Strong Computer skills a must.
