The CBHC Medical Specialist works with other CBHC medical staff to provide side by side interventions to clients and families with a focus on medical care. You provide medical screenings and medical care consultation as directed, while assisting with ensuring medical supports are in place to allow for continuum of care.

Responsibilities:

  • Provide education and consultation to individuals and staff regarding medical health, medication and medical topics.
  • Provide side by side interventions to clients and families focused on medical/psychiatric supports in the community, in the office, virtually and in residences.
  • Obtain phlebotomy training and certification in order to meet/perform phlebotomy and lab requirements within Eliot/CBHC policies and procedures.
  • Develop and conduct groups and interventions for individuals and families in general areas of health and wellness including nutrition, exercise, smoking cessation.
  • Provide medical screening of all assigned individuals. Obtain individual weight, height, BMI and other aspects related to metabolic syndrome to establish a baseline. Identify individual preference for making changes as applicable in regarding medical conditions, nutrition, smoking cessation, exercise or other areas related to overall health, utilizing motivational interviewing techniques.
  • Provide side by side interventions to individuals that focus on problem solving, skills training, modeling behavior, optimism and encouragement.
  • Perform direct medical care for individuals as appropriate and ordered by nurse manager/physician which may include monitoring of vitals, wound care, etc. This includes providing interventions specific to individual treatment plan for medical/health and wellness goal areas.
  • Review all medical documentation for each individual as assigned and provide medical care consultation and coordination as needed.
  • Provide assistance coordinating overall medical care for individuals and ensuring medical follow up is done as needed.
  • Develop working relationships with community medical providers and VNA related services in order to assist with coordinating medical care for individuals on team. Assist with acting as a liaison to communicate medical issues and concerns to medical providers and ensuring the treatment team has an understanding of concerns.

Qualifications:

  • Must be a licensed practical nurse, certified medical assistant, EMT-Advanced, EMT-Paramedic, and have at least two years of experience working with people with severe and persistent mental illnesses.
  • Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses and respect for their preferences for treatment.
  • Must have the ability to work on a team that provides flexible community based services.

Salary: $45,000 per year

Various schedules available.

Bilingual candidates preferred and eligible to receive a 6% differential if fluent.

Bonus Breakdown:

  • $1500 after 1 month of employment
  • $1500 after 6 months of employment
