If you are located in Flagstaff, AZ , you will have the flexibility to work remotely* as you take on some tough challenges. Travel 10% of the time*
*Primary Responsibilities: *
*

• Making outbound calls to assess members' current health status
• Visiting members in the hospital to prepare for transition home (varies by market)
• Identifying gaps or barriers in treatment plans
• Providing patient education to assist with self-management
• Providing clinic and provider education
• Interacting with Medical Directors on challenging cases
• Coordinating care for members
• Making referrals to outside sources
• Coordinating services as needed (home health, DME, etc.)
• Educating members on disease processes
• Encouraging members to make healthy lifestyle changes
• Documenting and tracking findings
• Making "welcome home" calls to ensure that discharged member receive the necessary services and resources

*Required Qualifications: *
*

• Current, unrestricted RN license in Iowa or compact license
• 3+ years clinical experience in a hospital, acute care, home health/hospice, direct care
• 2+ years of Case Management experience
• Experience working with clients in the community setting
• Demonstrate a proficiency in computer skills including Microsoft Word, with the ability to navigate a Windows environment
• Designated work space and access to install secure high speed Internet via cable / DSL in home
• Ability to travel up to 10% within a designated area
• Access to reliable transportation
• Currently resides in Arizona
• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifcations: *

• Bachelor's Degree
• Certified Case Manager (CCM)
• 5+ years experience as an RN
• Experience / exposure with discharge planning
• Background in managed care
• Solid organizational skills and multitasking abilities will be keys to success
