Case Manager Overview:
- In collaboration with the care team, case managers are responsible for assessing, planning, implementing, monitoring, and evaluating options and services needed to meet an individuals health needs.
- The focus for a case manager is to develop a patient-centric plan which assures quality care and cost-effective treatment options delivered in the most appropriate setting, while avoiding duplication and fragmentation of care.
- Specific responsibilities are dependent upon the targeted outcomes and strategy of the organization.
Qualifications:
- Education: Graduation from registered nursing program. Bachelors (or more advanced) Degree in Nursing.
- Licensure: Current and unrestricted license as a Registered Professional Nurse in the State(s) of which the nurse will practice.
- Certification/Registration: CCM or RN-BC professional case management certification preferred or attained within 36 months of hire.
