As Rhode Island’s largest health system and private employer, Lifespan is dedicated to improving the health of the people within our communities. We are seeking a part-time RN Case Manager to join us at the Rhode Island Hospital in Providence, RI. You will provide coordinated care support in collaboration with clinical healthcare teams across the patient care continuum and assist with patient pre-admission and post-hospital discharge.

Responsibilities:

• Participate in daily care rounds, collaborating with members of the patient’s healthcare team to evaluate, develop and implement the discharge plan and process
• Identify high-risk patients within the caseload for care management assessment screening and target interventions with the healthcare team
• Provide coordinated care support to facilitate and expedite patient care services
• Proactively build post-hospital referrals and send to the Transition Care Coordinator for timely discharge
• Develop appropriate patient care reports to ensure safe patient handovers occur as patients are transferred from one care area to the next
• Coordinate patient’s length of stay with the physician care team and patient and ensure the team is informed of insurance qualifiers that may affect the discharge plan
• Advocate for the patient and advise the patient regarding financial implications of their discharge plan
• Identify opportunities to improve patient flow and reduce service delays through problem resolution and follow-ups
• Participate in departmental and/or interdepartmental quality improvement activities and ongoing education-related professional activities

Qualifications:

• Graduate of a School of Nursing, Bachelor’s degree preferred
• Current RN license to practice in the State of Rhode Island
• Certification in Case Management by a national organization preferred, or to be obtained within 1 year of hire
• 3+ years of clinical experience, including recent experience with case management, case management patient navigation or discharge planning
• Demonstrated knowledge of nursing theory and practice with a clinical background within a defined content area
• Strong organization, communication and collaboration skills
