SUMMARY:

Family Service League is Long Island’s largest accredited human service agency. With more than 60 programs, we support over 50,000 individuals and families in need every year. Our services help individuals, children, and families to mobilize their strengths and improve the quality of their lives at home, in the workplace, and in the community.

Family Service League is seeking a part-time Case Manager for our Family Connect Program. Family Connect provides short term crises intervention services to children, adolescents, and their families to avoid the need for additional in-home services or assist with meeting immediate needs while waiting for these services. Areas of focus may include mental health, emotion regulation, education, and various other areas related to a young person's functioning.

RESPONSIBILITIES:

• Provide linkage, coordination, advocacy and referrals to youth and families.
• Provide direct service, including anger management and skill building to avert out of home placement.
• Conduct home visits throughout Suffolk County (some visits are currently virtual).
• Focus services on goal achievement surrounding factors that contribute to youth’s involvement with mental health services.
• Perform other duties as assigned.

QUALIFICATIONS:

• Bachelor's Degree in Social Work or related field required.
• At least 6 months of related experience required.
• Excellent interpersonal and verbal and written communication skills required.
• Proficient computer skills, including Microsoft Office required.
