Case Manager Coordinator is responsible for telephonically and face to face assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services, strategies, policies and programs are comprised of network management and clinical coverage policies. Contractual requirement and ratio to meet regulatory requirements.

Pay Range
The typical pay range for this role is:
Minimum: 19.50
Maximum: 40.10

Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.

Required Qualifications
  • 3+ years experience in behavioral health, social services, or human service field.
-Willing to travel 50 - 75% to meet members for face to face assessments.
  • Must possess a Virginia state Drivers license, a clean driving record and reliable transportation for field work in and around Roanoke, Martinsville, Bassett, Collinsville, Rocky Mount and Ferrum.

Preferred Qualifications
  • Case Management in an Integrated Model of Care
  • Managed Care Experience

Education
Bachelor's Degree or Non-licensed Master Level clinician required, with either degree being in behavioral health or human services preferred (Psychology, Social Work, Marriage and Family Therapy, Counseling)
