Job Title: ACM Coordinator
Department: Health Services - ACM

We are currently seeking a highly motivated Case Management Coordinator - Ambulatory Care. This role will report to a Supervisor - ACM and enable us to scale in the healthcare industry. This position will work remotely in Bay area.

What You'll Do:

• Responsible for member/provider outreach and notification (telephonic and/or written) utilizing department templates
• Responsible for assisting ACM nurses with member assessment and communication, which includes but is not limited to eligibility verification, requesting medical records, creating case, and documenting care activity
• Participate in all health plan and department meetings and trainings as applicable
• Assist with health plan audit preparation
• Perform other duties, projects and actions as assigned

Minimum Qualifications:

• 
  High school diploma OR GED
• Experience using Microsoft applications such as Word, Excel and Outlook

You're great for this role if:

• Previous experience with SNP Case Management
• Able to speak and understand Mandarin or Cantonese
• Experience working in a MSO/IPA/Health Plan

Environmental Job Requirements and Working Conditions:

• This position is remotely based in the U.S.
• Candidate should be based in the Bay area.
• The target pay range for this role is $16.50 - $28.50 per hour. This salary range represents our national target range for this role.
