ChristianaCare HomeHealth is seeking a Full Time Care Transitions Coordinator - Social Worker. The successful candidate will support New Castle County, DE; work day shift hours 8:30am-5pm, and an occasional weekend day or holiday based upon departmental needs.

Duties and Responsibilities:

  • Develop Relationships with referral sources, providers and other health care professionals.
  • Pre-admission evaluation and education of patients identified for home health to identify care needs, available resources, and validate information needed to ensure safe transfers across the care continuum.
  • Perform ICD-10 coding of referrals.
  • Coordinate with branch office manager to schedule start of care and supplies for services ordered by the provider as well as communication with referral sources and providers.
  • Identify patients appropriate for specialty programs and telehealth.

Requirements:

  • Bachelor's Degree in Social Work from a college/university accredited by the Council of Social Work.
  • 2 years of Social Work experience required; 1 year within health care, preferably in a post-acute setting.
  • Previous case management or discharge planning preferred.
