Join our amazing memory care team! Elaine's Hope Memory Care is in search of a dedicated and compassionate Care Manager to join our team!
The Care Manager assists in the leadership and direction of resident care areas and acts as a liaison and advocate for our residents and families ensuring their psychosocial needs are met. Supervises resident assistants to ensure care is provided according to regulatory requirements and department expectations.
Hours for the position are Monday - Friday from 8:00 AM - 4:30 PM.
Qualified candidates will have a bachelor's degree in social services or healthcare field and three years experience in long term care; memory care experience preferred.
Position is located at the Lutheran Home 7500 W North Ave, Wauwatosa, WI 53213.
