Summary
The Care Coordinator position is full-time and works out of our Yuba City, CA office, serving all of Sutter and Yuba County within our FSP program. FSP provides Specialty Mental Health Services that are integrated, collaborative and strength-based to youth up to age 15 with full scope Medi-Cal. The Care Coordinator will coordinate these Specialty Mental Health Services and/or community resources to meet the needs of our youth and their families.
Experience and/or Education
- A Master’s Degree from an accredited school in social work, education, public health, child development, psychology or related field is required.
- Prior crisis and case management experience is preferred.
- You must possess a State of California Driver License in good standing.
The estimated base pay for Care Coordinator I is $27.00 to $33.00 and for Care Coordinator II is $29.00 and $35.00, additional individual compensation may be available. Base pay is only a portion of the total rewards package, and a comprehensive benefit program is available for qualifying position.
