POSITION SUMMARY The Access to Care Coordinator is primarily responsible for assisting with patient care and engagement by promoting effective integration of patients and their connection to behavioral health services. This individual serves as an advocate for patient needs by coordinating care to minimize the wait for services, effectively assessing patient needs, and helping increase patient engagement in integrated services.

Monday - Friday (8am - 5pm)

This position is non-exempt, and reports to the Clinic Manager.

Essential Functions:

  • Gather information from patient and referral source to help identify appropriate resources and services
  • Assist in coordinating timely access to care for patients from intake and referral processes.
  • Verifies patient demographic and insurance information.
  • Collects co-payments, deductibles, and other financial responsibilities.
  • Ensure consent packets, and patient forms are completed and uploaded into patient chart.
  • Schedule interpreter services, and transportation for patient(s) as needed.
  • All scanning completed within 2 business days.
  • Accept perform other duties and responsibilities as assigned
  • Other duties as assigned

_ QUALIFICATIONS_

Education and Experience Requirements

  • High school diploma or equivalent required
  • 1-2 years’ professional experience in a behavioral health or integrated care office setting
  • Excellent communication and organizational skills
  • Problem solving, critical thinking, and conflict resolution oriented.
  • Collaboration between patients, and providers for excellent care
  • Knowledge of, and ability to use, electronic record keeping systems (EMR, billing software, insurance carrier portals, spreadsheets, etc.) preferred.
  • Bilingual English/Spanish fluency preferred

Job Type: Full-time

Pay: From $17.50 per hour
