Program: Intensive Case Manager (ICM)

Shift: First – Full-time (40 hours)

Job Hours: 7:00AM to 3:00PM - Monday through Friday; requires rotating weekends and/or additional coverage according to the needs of the home.

The Behavioral Health Intensive Case Manager Team Lead is responsible for providing supervision of Intensive Case Managers who will be providing services for adults with severe and persistent mental illness & addictive diseases through various View Point Health programs.


The Team Lead will provide services to cover Gwinnett, Rockdale, Newton, Dekalb and Fulton


The purpose for the position of Team Lead is to provide leadership & direction for 5 Case Managers, provide group and individual supervision for both teams, assist Supervisor in the development of policies and procedures to increase the efficiency and effectiveness of services to clients, and provide support to Case Managers, carry a minimal caseload of clients, and process incoming referrals for services.


ICM Team Lead and ICM providers will work closely with ACT, hospitals, Community Service Boards, Crisis Stabilization Units, Inpatient psychiatric units, jails coordinators, probation officers, other community partners/agencies, and other VPH programs. The Intensive Case Managers position will focus on the coordination of rehabilitation and recovery services within the agency and in the community for those clients who frequent crisis stabilization & community hospitals, repeat legal offenses, qualify as chronically homeless, and stepping down from ACT and other higher levels of care due to client behavioral health needs.


Flexibility in scheduled hours is necessary to provide effective in-home services throughout the day and into the evening hours. On-call rotations will be a necessary function of each Intensive Case Manager. The vast majority of services provided to clients will be in the client’s residence and their community. It will be the ICM Team Leads responsibility to coordinate weekly team meetings (1 meeting per week per team) to assess every client’s needs individually, develop/update treatment plans, and monitor client progress.


This position requires the ability to work in a fast pace setting, a passion to work with those with Behavioral Health needs, and the ability to work closely and effectively with a team of professionals. Reliable transportation and driving abilities is imperative.

Duties & Responsibilities:

  • Supervise, support, coordinate, organize, & implement the responsibilities of the Intense Case Management position.
  • Develop and implement policies and procedures to increase the efficiency and effectiveness of services to clients.
  • Conduct quarterly reviews of supervisees’ performance & conduct performance evaluations mid-year & annually for each ICM.
  • Complete Hiring Packets and Conduct Interviews as necessary
  • Attend Program Management meetings & Trainings
  • Track and monitor training needs of supervisees.
  • Carry a small caseload of clients and provide support and coverage for employees as needed.
  • Meet tem lead productivity standards & document within 24 hours.
  • Conduct weekly team meetings with ICM
  • Crisis assessments for clients in crisis to assess for safety and conduct 1013 if necessary.
  • Partake in and manage ICM on-call rotation
  • Conduct 1 home visit per week/per client within their community setting or within client’s current residence.
  • Interview clients, family members, and/or support system(s) to assess level of functioning, current Strengths, Needs, Abilities, and Preferences and secure information relative to program requirements for receiving services and recommending appropriate level of care.
  • Licensed staff is to conduct Behavioral Health Assessments and update Treatment plans to reflect needed services for authorization.
  • Participate in weekly treatment team meetings to assist in the development of treatment plans with appropriate therapeutic interventions, notifies professionals of any unusual observations or incidents, client progress, and follow up.
  • Develops and maintains a network of community contacts. Facilitates referrals and linkages to community services and communication among client’s service providers with VPH to assure a continuum of services.
  • Provide linkage, transport clients, and assist/coach clients in accessing & obtaining community resources.
  • Establishes linkage with community resources (jails, hospitals, shelters, DFCS, etc.) to assist with discharge planning and a smooth transition to View Point Health Services.
  • Process referrals from other inter-agency providers & Community partners & check against strict requirements for ICM services for acceptance or denial.
  • Each ICM provider is to maintain an individual case load of 25 to 30 clients each.
  • Write precise, accurate, and clear progress notes in our electronic Care Logic system in a timely manner. Maintain required records and information in each client’s file. Prepares accurate, complete timely documentation (within 24 hours) of case notes and billing information to meet agency and state standards and provide effective service delivery across a broad range of services.
  • Provide individual skills training as indicated in the client’s treatment plan including but not limited to independent living skills, coping skills, interview skills, symptom management skills, anger management skills, assertiveness skills, advocacy skills, hygiene skills, and many other daily living skills.
  • Recognize the need for crisis intervention and notifies supervisor and professional staff of escalated situations.
  • Under professional guidance, provides individual and group supportive counseling to clients and families concerning their needs and problems, and issues with personal and social development.


Requirements/Competencies:

  • Must pass a pre-employment drug screen and subject to random drug screens
  • Must have valid Driver’s License and a clean Motor Vehicle Record in accordance with company policy
  • Must pass a criminal background check (including fingerprinting)
  • Requires TB Evaluation at pre-employment and annually
  • Must successfully complete VPH New Hire Trainings, cross train at other VPH MH Homes, and complete 16 hours of training annually
  • Must be able to lift 20 pounds
  • Requires long periods of sitting/standing
  • Must have the following technical skills – Word, Excel and Electronic Health Record (Care Logic)
  • Must be willing to transport residents in company van
  • Must demonstrate the ability to maintain professional boundaries with clients in an intense, specialized residential treatment setting
  • Must demonstrate the ability to appropriately redirect challenging behaviors of clients in an intense, specialized residential treatment setting
  • Must be able to lift and carry groceries and supplies and move furniture for cleaning
  • Must be able to assist disabled individuals on/offvehicle and secure seatbelts
  • Must be able to use a gait belt and mechanical lift to lift and position individuals, as needed
  • Must be willing to travel between Gwinnett, Rockdale and Newton counties to provide support for clients in the Mental Health Community or to support coverage at another VPH MH home, when needed
