We are seeking a dynamic, patient-centered nurse practitioner to help fulfill our vision.

Position offers competitive annual salary guarantee. Prefer candidates with two or more years of provider experience. Willing to consider new graduates with strong previous experience in elder care. Adult/gerontology training a plus. Must be committed to a team approach to care. Providers work closely with hospitalists, emergency medicine, hospice and palliative medicine, primary care, and specialists to ensure continuity of care.
