The Division of Geriatrics seeks a full-time Advanced Practice Practitioner (APP)/(Nurse Practitioner II) to support the Dementia Care Aware (DCA) state-funded initiative. The successful candidate will be able to skillfully navigate the unique needs of the program, work independently in a fast-paced environment, collaborate effectively with key stakeholders, and play a key role in program implementation and sustainability. The APP will work closely with the leadership team and their DCA clinical team to support primary care clinics within the San Francisco Health Network (SFHN) to implement the DCA program and create dementia-friendly care in the network. The APP will lead a small team of clinical social workers/specialists and administrative staff to perform cognitive assessments, order diagnostic testing and place referrals, as well as determine and initiate care plans for patients with cognitive impairment. The APP will also work with their DCA team to educate existing clinical staff in the network to improve their capacity to do cognitive assessments and care for people with symptoms. Clinical work will be by phone, video, and in person. Training in the assessment of cognitive symptoms and function, working with people living with dementia, and working with caregivers will be available. The APP will also educate and counsel patients, families, and caregivers about their cognitive health and provide instruction on the treatment plan. The APP will be able to encourage and promote activities for improved clinical care around brain health and perform other duties deemed necessary for overall program success. Those with strong leadership skills and experience in dementia are highly preferred.

Key Responsibilities will be delineated as:

25% - Assess patients and caregivers with Cognitive Health Assessment and develop and initiate care plans for patients with positive screenings.
25% - Educate and train clinical staff on resources for older adults.
20% - Supervise patient navigators on the team and work with clinical team members (clinical social workers) with care planning and patient navigation for older adults with cognitive impairment.
20% - Provide education and counseling to patients, families, and caregivers on cognitive health.
10% - Communicate and collaborate with DCA program staff. Work with DCA program manager on implementing the program, identifying areas for improvement in the program, and creating materials for the primary care network to use in the development of care protocols for people with cognitive symptoms and dementia and their care partners.

A Nurse Practitioner is an advanced practice registered nurse who possesses the advanced education, knowledge, clinical skills, and experience necessary to provide expert advanced nursing care in their area of specialization. They will be guided by and follow the policies and standardized procedures, rules and regulations, requirement of supervision under CA statute, and bylaws of the UCSF Medical Center, some Nursing, Medical Staff, and any unique policies related to the provision of patient services. In California, nurse practitioners (APRNs) can practice, bill, and provide independent health care services under a standardized procedure in collaboration with a supervising physician.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% - 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.

For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

Department Description

Nurse Practitioner (NP) position is assigned to the Clinical Practice in: UCSF School of Medicine, Department of Medicine, Division of Geriatrics.

The Division of Geriatrics’ work is grounded in a deep commitment to and respect for older persons. We are specialists in aging. Our expertise is in optimizing the health of older individuals. We work in the community and on virtually every UCSF campus – Parnassus, San Francisco Veterans Affairs Medical Center, Zuckerberg San Francisco General Hospital, Mission Bay, and UCSF Center for Geriatric Care. The Division has over 130 employees including faculty, volunteer clinical faculty, fellows, staff, student interns and volunteers. Dementia Care Aware, is a state-wide initiative for the Department of Health Care Services led by UCSF’s Program Directors. Dementia Care Aware is a multi-year, multi-site, statewide initiative aiming to improve the lives of people living with dementia, and the people who support them, by rapidly improving the ability of primary care for underserved populations to assess and care plan for people living with dementia. The program is specifically focused on improving dementia care for Medi-Cal recipients and thereby reaching underserved patients living with dementia, such as those who are non-English speaking or are experiencing adverse social determinants of health. In San Francisco, we are partnering with the San Francisco Health Network’s primary care services to offer health systems implementation support to realize the DCA program and change practice. The San Francisco Health Network provides comprehensive health care services to SF residents who are low-income, has 14 primary care clinics, and serves 60,000 patients a year.

Required Qualifications

  • Work or educational experience in geriatrics, aging, safety net, or other closely related field(s)
  • Demonstrated ability to supervise clinical staff
  • Exceptional oral and written communication skills targeting a diverse audience
  • Strong ability to think creatively and independently on concepts requiring advanced analytical skills
  • Strong interpersonal skills and ability to work collaboratively with all team members
  • Experience in developing care plans and requesting or providing consultative services
  • Must have completed a BRN recognized graduate master’s degree program as a nurse practitioner.
  • Must have at least 2 years of adult primary care practice experience as a family nurse practitioner (FNP or ANP) in a diverse patient population
  • Must have experience with clinical documentation in an electronic health record
  • Must have provided patient care for older adults as part of their clinical experience
  • Able to interpret ordered tests, procedures, and radiographic studies
  • Able to adhere to all policies and procedures in terms of HIPPA/Communication/Privacy/Code of Conduct within the San Francisco Health
  • Network, which includes meeting clinical encounter closure requirements.

Preferred Qualifications

  • Geriatrics experience (clinical care for older adults) inpatient or outpatient
  • DEA-X waiver
  • Ability to qualify as trusted interpreter in Spanish, Chinese languages, or Tagalog.
  • Experience using EPIC
  • Ability to travel within California to achieve program goals
  • Experience and/or training in dementia

License/Certification

  • Must hold current national certification from an accepted certifying agency and provide by first day of work.
  • Must hold a current and active RN and NP and Furnishing license in California.
  • Must have completed a BRN recognized graduate master’s degree program as a nurse practitioner.
  • Must obtain an active and/or transferable DEA license to California and provide evidence by 90th day of clinical practice. (It is the responsibility of the employee to transfer their DEA license to their new place of employment if from out of state, via online DEA portal and provide evidence of transfer).
  • Must have submitted enrollment with CMS Medicare and Medicaid through credentialing by first clinical day. CCS enrollment as appropriate for pediatric environments.
  • Must hold current AHA BLS as minimum requirement
  • State regulation requires that you have a supervising physician to practice
  • Incumbents with less than 6 months of NP experience will be hired as a Nurse Practitioner 1
