Waban is seeking a skilled, motivated, and highly organized professional to join our growing Adult Case Management team. Waban Adult Case Management Program provides support to adults with disabilities and their families. The Adult Case Manager is an advocate; ensuring choice, satisfaction with services and monitors the individuals health and safety.

The Adult Case Manager assists the individual and their team to identify goals and develop a plan of care. The Adult Case Manager is responsible for referral and monitoring of services. Case Managers work closely with a multidisciplinary team of professionals. The Case Manager coordinates and advocates for services identified in the plan, monitors progress, and evaluates the effectiveness of services.

*This position requires travel in the community* Available coverage areas include York and/or Cumberland County

Requirements: Must have a reliable vehicle with proper insurance, a valid driver’s license from state of residency and a clean driving record.

Must be proficient in speaking, reading and writing English with strong interpersonal communication skills.

Must be able to complete agency and State mandated training programs with proficiency.

Must be able to use a computer to enter time & attendance, document all aspects of service delivery, complete trainings, and communicate effectively with supervisor, co-workers, families, and other professionals.

Minimum Qualifications: Qualified candidates will have a Bachelor’s Degree in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Nursing, or closely related field. Minimum one year of experience working with children, teens or adults with intellectual or mental health disabilities.
