SCOPE: This position is expected to assist individuals on the Adult Autism Waiver with the location, coordination, and monitoring of needed services and supports.

  • $27.00/hour
  • Remote Position in PA
  • Area of coverage: Northampton, Montgomery, Berks, Delaware, Lancaster counties.
  • Part-Time: 20 hrs/wk
  • Mon-Fri, should be flexible to accommodate case load needs

ESSENTIAL FUNCTIONS:

Job duties include :

  • Conduct annual assessments, including Scales of Independent Behavior-Revised, Parent Stress Scale, and Individual Support Plan
  • Develop an Individual Support Plan, using a person centered approach, working with a planning team. Work with institutional discharge planners as needed.
  • Conduct ongoing monitoring of the services included in a individual’s ISP, meeting the individual in person quarterly, and contact the individual, guardian, or representative at minimum once monthly.
  • Maintain case records using HCSIS.
  • At least annually, reconvene the planning team for a comprehensive review of the ISP and at least annual collect information to inform level of care reevaluations by ODP staff.
  • Develop and maintain a professional and friendly working relationship with staff and others, fostering a positive work environment and employee morale.

REQUIREMENTS:

  • At least a Bachelor’s degree in Education, Psychology, Social Work, or other related social sciences.
  • Have either 1) at least three years experience providing case management for people with disabilities, or 2) at least three years experience working with people with autism spectrum disorders.
  • Complete required training developed by Bureau of Supports for Autism and Special Populations, including training in needs assessment and person-centered planning.
  • Knowledge of needs assessment, applied behavioral analysis, and person-centered planning.
  • Ability to meet CRI and program background check requirements
