POSITION SUMMARY

The Care Coordinator is responsible for the majority of discharges on units within VVHS. The Care Coordinator completes individualized discharge and safety plans. The Care Coordinator provides patients with a review of treatment options beyond VVHS. The Care Coordinator represents VVHS in coordination of treatment follow up for patients within the community. The Care Coordinator works closely with other members of the social services team from the time of intake to plan for an appropriate discharge plan.
