Nurse Case Managers shall provide services in support of Individual Medical Readiness to the Army Reserve-Medical Management Center (AR-MMC). The Administrative Nurse will work in conjunction with other health care providers, professionals, and non-contract personnel. No clinical or direct patient care.

Education and Experience:

  • Registered Nurse (RN) License with a minimum of 1 year of case management experience in either inpatient or outpatient setting.
  • Licensed Practical Nurse (LPN) License prefer at least 1 year of administrative experience.
  • Prefer prior military experience.
  • Current Basic Life Support (BLS) certification from the American Heart Association or Red Cross.

Knowledge, Skills, and Abilities:

  • Computer skills to enable them to develop working knowledge of e-Case, MODS/MEDPROS/MEDCHART, AHLTA, Medical Readiness Portal, MEB Prep, JLV, eMMPS, HAIMS, RLAS, IWS, iPERMS, CSMM and RHRP.
  • Be proficient using Microsoft Word, Excel, PowerPoint, and Outlook.
  • Ability to communicate effectively orally and in writing.
  • COVID vaccination will be required. .

Additional Requirements Include:

  • U.S. Citizenship (certified copy of birth certificate or naturalization papers).
  • Strong communication skills, problem solving skills, solid background in general nursing.
  • Strong organizational skills and ability to work independently and collaboratively with the Case Management Team and designated military personnel.

WORK SCHEDULE & LOCATION(S)

  • Normal duty hours will be from 0800-1700 with a 1 hour lunch break with the flexibility in work hours from 0730 – 1700 and lunch break 1/2 - 1 hour. No overtime is permitted.

ESSENTIAL JOB FUNCTIONS

  • The Nurse Case Managers will work in collaboration with the AR-MMC to facilitate identification of the MNR population, ensure assessment of the MNR population and will coordinate with unit leaders and available medical options to determine what the MNR Soldiers need. Assist Soldiers in managing referrals for care and in becoming medically ready (MR) or begin medical boarding packet building. Provide guidance and assistance to Soldiers who require referrals for medical treatment or care and assist soldiers with individual medical readiness (IMR) support to include identification and tracking of soldiers who are not medically ready (MR). Nurse Case Managers will have access to Medical Operational Data System (MODS), Medical Protection System (MEDPROS), Medical Readiness Portal, Medical Evaluation Board (MEB) Prep, Joint Legacy Viewer (JLV), Electronic Medical Management Processing System (eMMPS), Healthcare Artifact and Image Management Solution (HAIMS), Regional Level Application Software (RLAS), Interactive Web Response System (IWRS), Personnel Electronic Records Management System (iPERMS), Commander’s Strength Management Module (CSMM), Medical Electronic Data (for) Care History And Readiness Tracking (MEDCHART) Health Readiness Record (HRR), Armed Forces Health Longitudinal Technology Application (AHLTA, Army Knowledge Online (AKO) and Reserve Health Readiness Program (RHRP). Nurses shall be familiar with and understand regulations pertinent to managing Soldiers who have medical issues and medical readiness without deviation from standard. Collaborate with DIV(R) Surgeon, Chief of AR-MMC or designee when assistance is required.
  • Communicate and create record for every Soldier within 7 days of receipt of receipt of case from the Team Lead. Priority communication method will be telephone calls with emails sent if the soldiers is unavailable.
  • Document in soldier’s record at a minimum of every 30 days.
  • Ensure documentation is received and filed appropriately.
  • Manage medically not ready Soldier’s case from receipt of case referral or notification through case closure, as required.
  • Perform intake interview with Soldier and establish communication with command. Ensure Soldiers understand his/her responsibilities for making and keeping appointments and provide the Nurse with medical documentation.
  • The Nurse is responsible for educating the Soldier about their privacy rights and how their information may be used and obtains consent for release of medical information.
  • The Nurse will provide a monthly case report to their assigned Team Lead and AR-MMC designated representative. The report should include number of cases opened, closed, referrals made, important unresolved issues and the number of cases identified needing a medical evaluation board. The Nurse will also be required to submit a minimum of 2 packets per month is required after the initial 90 days.
  • Establish and maintain a usable schedule, track and report case status, evaluate all deliverables to ensure high quality, ensure packets meet internal and external expectations, customize reports as required by Team Lead or government personnel.
  • Performs other duties as assigned.

Pay: $24.00 - $38.00 per hour
