Position Summary:

The Health and Wellness position requires an understanding and commitment to the mission and core values of Charter Senior

Living by exhibiting professional and ethical work practices. The Health and Wellness Director is responsible for the oversight and management of staff and residents with the primary focus of serving the healthcare needs of Charter Senior Living residents in accordance with Federal, State regulations and standards. The Health and Wellness Director assures the quality care of all residents in conjunction with the Executive Director, Resident Care Coordinator (if applicable) and other department managers.

Essential Duties:

  • Directly oversee/assist with the delivery of care to Assisted Living/Memory Care residents to ensure that emotional, physical, psychological and safety needs are met through all program services and activities.
  • Adhere to all policies and procedures of Charter Senior Living.
  • Work with staff to ensure that high standards of care and customer service are delivered as per resident preferences and plans of care.
  • Complete the comprehensive evaluation on potential residents and current residents for Assisted Living/Memory Care to develop a level of care per state regulatory guidelines.
  • Monitor documentation systems to ensure that all regulatory timeframes for review of evaluations and service plans are met and that resident needs are met.
  • Monitor documentation from home health, hospice, and ancillary care providers, to assure appropriate communication of services provided and to assure needs of the resident are being met.
  • Advise management personnel to be aware of resident health issues for which they may need to observe and report or take specific actions.
  • Provide oversight and management to the medication management program within the community.
  • Responsible for monitoring of key quality performance metrics within their department; collaboration with clinical leaders on plan development to achieve improvement in quality of care and compliance outcomes.
  • Routine communication and collaboration with the Regional Director of Health and Wellness on clinical performance metrics, regulatory compliance, and quality of care enhancements.
  • In conjunction with the Executive Director, hire, train, supervise, and utilize progressive discipline for care staff as conditions arise.
  • Responsible for assuring that department is appropriately staffed, per budget, to appropriately meet the health and well- being of residents which may include management and/or oversight of the staffing schedule.
  • Supervise Wellness staff, including their schedule, on-time attendance, performance as a team member, individual performance of daily duties and responsibilities and attitude and compassion with residents.
  • Perform required and as-needed evaluations with appropriate feedback and performance plans.
  • Plan and schedule staff training programs and in-services for the Assisted Living/Memory Care staff as indicated.
  • Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
  • Must maintain and encourage a positive work atmosphere and communicating in a manner that promotes cooperation and quality experiences for co-workers, managers, and others.
  • Must demonstrate empathy and compassion to connect with staff and residents. Must be inspired to serve the needs of our customers.
  • Promote a positive customer service experience by maintaining a clean work environment.

  • Contribute to strategic management of Charter Senior Living by participating in senior management planning sessions and consulting with other department heads.
  • If required by the state, will perform the duties of the delegating nurse at the community if certified as a Case

Manager/Delegating Nurse.

  • Performs other duties as assigned.

Physical Demands and Working Conditions

  • Ability to lift/carry up to 50 pounds. 80 pounds of pressure may be placed on the associate through transfers using transfer belts or mechanical assists.
  • Must be able to stand, walk, stoop and/or bend for periods of up to eight hours with breaks as provided for in the employee handbook.
  • Possible exposure to communicable diseases, infectious diseases, and bloodborne pathogens.

Other Requirements:

  • Has familiarity and willingness to work with elderly population. Previous experience preferred, but not necessary.
  • Demonstrated ability to appropriately delegate responsibility with necessary authority without relinquishing his/her overall responsibility and accountability for the delegated task.
  • Demonstrated ability to identify problems in a timely manner, develop effective resolutions to those problems and ensure implementation of the proposed resolution, confirm results, and revise, as necessary.

Qualifications:

  • Must be licensed as a Licensed Practical Nurse or Registered Nurse with a license in good standing.
  • Maintains active CPR certification
  • Maintains CEUs as per state license renewal requirements
  • Minimum of one (1) year nurse management experience, including hiring, coaching, performance management and daily clinical operation supervision
  • Knowledge of applied nursing practice and techniques in accordance with federal, state, and local regulations
  • Outstanding assessment, survey management, medication management, and follow-up
