We are currently looking for a Registered Nurse to join our team on a per diem basis as a Triage Level 3 Nurse within our Hospice division. This position would be responsible for providing skilled nursing care to patients with life threatening and life limiting illnesses and their families.

Number of days need per week: 2

Responsibilities:
1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
2. Triages phone calls and schedules visits on a daily basis
3. Collaborates with physicians and other health care providers regarding client’s needs
4. Provides education and serves as a resource for field staff
5. Makes home visits as needed
6. Monitors quality and appropriateness of services provided to ensure patient satisfaction
7. Adheres to agency infection control and safety policies
8. Adheres to the organization’s policy in regards to absenteeism and appearance
Qualifications:
1. RN, BSN or working towards licensure in NJ
2. 2+ years of community health experience with 1+ year of prior experience with the agency
3. Excellent verbal and written communication skills
4. Strong organizational and interpersonal skills
5. Knowledge of reimbursement and OASIS requirements
6. Valid driver’s license

Working Conditions/Physical Demand
: Working indoors and outdoors with phone and computer use; must be able to lift fifty pounds (50 lbs.)
