The Case Manager provides screening, assessment, advocacy and ongoing case management for homeless individuals placed in shelter. The Case Manager helps unhoused guest establish and work on short term goals to help them stabilize their lives and move toward securing more stable housing. Provide support services such as: information and referral, goal setting, problem solving and linkages to any needed wrap around services.
Essential Functions

  • Collaborate with management and staff to ensure the goals and vision of City Hope St. Louis and its mission to cure homelessness.
  • Foster a solution-focused environment that is fair and consistent, with the intention to engage guests.
  • Maintain a professional culture that promotes teamwork with empathy for guests.
  • Ensure data integrity and accurate reporting
  • Review guest intake data for accuracy and completion
  • Ensure case notes are up-to-date and accurate
  • Provide participant intake, screening, assessments, placement and ongoing goal setting and monitoring.
  • Assist customers to establish more stable housing by helping them develop problem solving and goal setting skills.
  • Assist customers in determining their needs in all areas, including, but not limited to employment, education and financial needs.
  • Provide information and referral and assistance to drop-in and telephone customers.
  • Maintain accurate and up to date customer records and case notes (in HMIS and on paper).
  • Develop and maintain good relations with customers and other community resource providers.
  • Serve as customer advocate for needed services.
  • Proven ability to be flexible and to work in a fast-changing environment.
  • Be proficient with basic computer skills, including Word and Excel, and a willingness and ability to learn additional databases.
  • Ensure that each guest information is accurate and up-to-date in the HMIS system.

  • Microsoft Office
  • HMIS certified (preferred) ; if not HMIS certified must be willing to complete HMIS training and pass test within the 45 days of hire.

Work Environment
This job primarily operates in both a professional office and in shelter environment.
This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit; stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move objects up to 10 pounds and occasionally lift and/or move objects up to 25 pounds unassisted. The employee must be able to self-transfer, climb stairs and move throughout the facilities, between buildings, and around the property. Close visual acuity to prepare and analyze reports and data. Visual acuity and physical ability to operate a motor vehicle.

Qualifications:
(Required) Bachelors degree or a minimum of two-years relevant human services experience working with the under-served, impoverished and/or the homeless or an equivalent combination of education and experience; Intermediate to advanced computer skills in Word, Excel and Outlook;

Evening and weekend work may be occasionally required as job duties demand.
