Duties

Duties may include, but are not limited to:

• The clinical Social Worker conducts psychosocial/mental health assessments of Veterans independently and/or in collaboration with the interdisciplinary treatment team, the Veteran, family members, caregivers, and/or significant others. The goal of the assessment is to identify needs and formulate a patient centered treatment plan with the Veteran to meet those needs.
• The clinical Social Worker is knowledgeable of a range of treatment modalities and has the skills required to provide the appropriate level of care/treatment through psychosocial treatment and evidence based clinical/psychotherapeutic interventions.
• The clinical Social Worker must be knowledgeable of mental health conditions and how they acutely and chronically impact Veterans day to day lives.
• The clinical Social Worker must understand how to properly diagnosis mental health conditions using the most current version of the DSM and this understanding must be accurately demonstrated through clinical documentation in the electronic health record.
• It is the responsibility of the Social Worker, working within the interdisciplinary treatment team, to share in the development of a treatment plan with the Veteran based on this assessment.
• The clinical Social Worker is responsible for the development of a patient centered treatment plan and developing achievable treatment goals with the Veteran, and when appropriate, the family members/caregivers and collaborating with all additional interdisciplinary team providers.
• The treatment plan will be documented in approved VA treatment planning software and in the electronic health record.
• The clinical Social Worker is responsible for planning and organizing work, coordinating with clinical interdisciplinary team members, other professional and clinical staff, and management personnel.
• The clinical Social Worker is responsible for providing, reviewing and coordinating all phases of the continuing care for Veterans served.
• Throughout the course of treatment, the clinical Social Worker is the subject matter expert on VA and/or community resources.
• The clinical Social Worker will collaborate with other service providers in reassessing the Veteran's needs.
• The clinical Social Worker is responsible for educating the Veteran and family members/caregivers on the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of his surrogate decision maker.
• The clinical Social Worker will ensure discussion on resources and referrals to current and viable resources to best meet their psychosocial and mental health needs enhancing the care/treatment provided.
• The clinical Social Worker recognizes and understands the intimidation of bureaucracy and will act as an advocate when needed to serve the best interest of the Veteran and family members/caregivers. Additionally, the clinical Social Worker will educate and encourage the veterans to advocate on his/her own behalf, thus fostering a sense of independence and empowerment.
• The clinical Social Worker will conduct clinical crisis assessments, develop and execute crisis management treatment plans in a timely, effective and efficient manner.
• This may include coordinating voluntary or involuntary admission to acute mental health inpatient units in the event off a medical or psychiatric emergency situation.
• The clinical Social Worker will follow the policy/protocol of his/her respective mental health unit/program, NF/SGVHS, VISN, and the State of Florida.
• The Social Worker has access to multiple resources to meet the demands of a crisis and the needs of the Veteran and their family members/caregivers.
• Work will be performed appropriate to level of licensure and be clearly and accurately documented in the electronic health record.
