The Home Health Social Worker 2 provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illness. The Home Health Social Worker 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
Responsibilities
The Home Health Social Worker 2 services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management of interventions designed to promote, health, prevent disease, and address barriers to access to healthcare. Bachelor's degree in Social Work, and state licensure required. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.
Requirements
- Master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Social Worker licensure in the state of practice if required by state law or regulation
- Valid driver’s license, auto insurance and reliable transportation.
- Proof of current CPR
- Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice
- Knowledge of and the ability to assist with discharge planning needs, and to obtain community resources (housing, shelter, funeral/memorial service arrangements, legal, information and referral, state/federal financial and medication programs, and eligibility.
