Position Overview:

The Social Worker is responsible for helping the client and/or their personal support network to understand and adjust to their medical and social treatment plan. The Social Worker assists clients in identifying resources, maintaining and improving their ability to manage their everyday physical, mental, and psychosocial needs, and help them to make decisions based on those abilities. The Social Worker also supports clients to prioritize and make decisions about future medical decisions related to disease progression. The Social Worker strives to promote patient and family wellness, improved care outcomes, and access to appropriate clinic and community resources.

Essential Duties & Responsibilities:
  • Facilitates information flow regarding the care and overall community needs of patients, families, team members, other allied staff, and community resources.
  • Thinks innovatively regarding patients and how their health care needs can be met at the community level; utilizing available resources as appropriate to assure patient's social needs are fostered.
  • Responsible for completion of adequate assessments, documentation, and other necessary information relevant to the patient’s care management continuum.
  • Identifies, assesses, and appropriately manages and refers possible victims of abuse, neglect, and other potential exploitation.
  • Advises patients of their rights, including the right to make decisions about their care (e.g., the right to accept or reject medical procedures).
  • Participates in and supports continuous improvement initiatives. Utilizes CSI methodologies to decrease costs & minimize waste while improving quality and satisfaction.
  • Provides in-home, community-based bio-psychosocial assessment of the patient, family, and social support system. Promotes an atmosphere of support to the patient in the community that enables improved quality of life, competence, and control. Formulates goals, care plans, and interventions for clients who are at risk of frequent hospitalizations and emergency room visits as determined through initial assessment by the Provider Care teams and the CCN in order to prevent hospital admissions, visits to emergency care that are potentially medically unnecessary, and to support patients/clients in their homes. Collaborates with the CCN Nurse Navigator(s) on a frequent basis and with all members of the CCN team.
  • Provides interventions such as psycho-education, skills training, crisis intervention, and problem-solving counseling, as well as social support, and connection to externally available services to support needs. Develops goal-oriented care plans with clients to reduce the effects of chronic conditions and/or co-occurring disorders, such as mental health and chemical dependency issues.
  • Advocates for clients and family members with medical systems and community to help with financial resources, housing, medical care, and other services. Advocates for and supports patients and their existing support network(s).
Essential Skills and Experience:
  • Bachelor’s Degree in Social Work from a program accredited by the Council on Social Work Education.
  • Minimum of 3 years’ experience is required
  • Current CPR Certification
  • Current licensure as LSW in Minnesota is required. May need to obtain a Wisconsin license within 6 months of hire based on client need
  • Certification in Advanced Directive Counseling within one year of employment
  • Basic computer skills: Word, Outlook, Patient Information Database (Cerner, EMR, GE Centricity)
  • Knowledge of crisis intervention, motivational interviewing, advance care planning, and advanced communication skills are required

Nonessential Skills and Experience:

  • Master’s Degree in Social Work or related specialty is preferred
  • Experience in community social work, mental health, and chemical dependency is preferred
